医保预付制度下静脉用药集中调配中心药师的作用探讨

Discussion on the Role of Pharmacists in Intravenous Drug Centralized Allocation Centers Under the Prepayment System of Medical Insurance

目的:探讨静脉用药集中调配中心药师在医保预付制度下的作用。方法:选取2020年9—12月经过静脉集中调配中心药师审核的2030例参保患者5600条处方作为观察组,另选取2015年9—12月没有经过静脉集中调配中心药师审核的2010例参保患者4650条处方作为对照组,比较两组住院费用与医保费及参保不合理处方情况。结果:观察组住院总费用、自付费用、拒付人数低于对照组、医保费用高于对照组,差异有统计学意义(P<0.05);观察组不合理出方法及不合理大处方数、抗菌药物使用率、抗菌药物使用强度低于对照组,差异有统计学意义(P<0.05)。结论:在医保预付制度下,静脉用药集中调配中心药师对合理用药起到重要作用,能够降低参保住院患者静脉用药的经济费用。

Objective: To explore the role of pharmacists in centralized drug dispensing system under the prepayment system of medical insurance. Methods: A total of 5 600 prescriptions of 2 030 insured patients who were examined by pharmacists from the central venous distribution center from September to December 2020 as the observation group, and the other 4 650 prescriptions of 2 010 insured patients who had not been approved by the central pharmacy pharmacists September to December 2015 as the control group, and the hospital expenses, medical insurance premiums and unreasonable prescriptions were compared between the two groups. Results: The total hospitalization expenses, self paid expenses and the number of refusing to pay in the observation group were lower than those in the control group, and the medical insurance expenses was higher than those in the control group, and the difference was statistically significant(P<0.05). The unreasonable methods and unreasonable prescriptions, the use rate of antibiotics and the use intensity of antibiotics in the observation group were lower than those in the control group, and the difference was statistically significant(P<0.05). Conclusion: Under the prepayment system of medicare, pharmacists in the central pharmacy of intravenous drug use play an important role in rational drug use, and can reduce the economic cost of intravenous medication for insured inpatients.
