牡丹散加味联合抗生素与裸花紫珠片治疗气滞血瘀型盆腔炎性疾病后遗症临床研究

Clinical Study on Modified Mudan Powder Combined with Antibiotics and Luohua Zizhu Tablets for Sequelae of Pelvic Inflammatory Disease of Blood Stasis Due to Qi Stagnation Type

目的:观察牡丹散加味联合抗生素与裸花紫珠片治疗气滞血瘀型盆腔炎性疾病后遗症的疗效。方法:将纳入的94例盆腔炎性疾病后遗症气滞血瘀证患者按随机数字表法分为对照组和观察组各47例。2组患者均服用奥硝唑片、左氧氟沙星片5 d,对照组加予裸花紫珠片治疗,观察组在对照组治疗基础上给予牡丹散加味治疗,2组均连续治疗3个月。治疗前、治疗3个月后检测2组患者的白细胞计数值、中性粒细胞计数值及血清肿瘤坏死因子(TNF)-α、白细胞介素(IL)-6水平,评定气滞血瘀证症状评分。比较2组的临床疗效。结果:治疗3个月后,观察组临床疗效总有效率高于对照组(P<0.05)。2组白细胞计数值、中性粒细胞计数值均较治疗前降低(P<0.01),观察组白细胞计数值、中性粒细胞计数值均低于对照组(P<0.05)。2组下腹疼痛、月经不调、经行腹痛加重、乳房胀痛评分均较治疗前降低(P<0.01),观察组4项症状评分均低于对照组(P<0.01)。2组血清TNF-α、IL-6水平均较治疗前降低(P<0.01),观察组血清TNF-α、IL-6水平均低于对照组(P<0.01)。结论:牡丹散加味联合抗生素与裸花紫珠片治疗气滞血瘀型盆腔炎性疾病后遗症可有效缓解症状、减轻炎症,疗效较为显著。

Objective:To observe the curative effect of modified Mudan powder combined with antibiotics and Luohua Zizhu tablets for sequelae of pelvic inflammatory disease of blood stasis due to qi stagnation type.Methods:A total of 94 cases of enrolled patients with sequelae of pelvic inflammatory disease with blood stasis due to qi stagnation syndrome were divided into the control group and the observation group according to the random number table method,47 cases in each group.Both groups were given oral administration of ornidazole tablets and levofloxacin tablets,and the treatment lasted for five days.The control group was additionally given Luohua Zizhu tablets,and the observation group was additionally given modified Mudan powder and Luohua Zizhu tablets.Both groups were continuously treated for three months.Before treatment and after three-month treatment,the values of white blood cell count and neutrophil count as well as levels of tumor necrosis factor-α(TNF-α)and interleukin-6(IL-6)were detected in the two groups.The symptom score of blood stasis due to qi stagnation syndrome was evaluated.The clinical effect was compared between the two groups.Results:After threemonth treatment,the total clinical effective rate in the observation group was higher than that in the control group(P<0.05).In the two groups,white blood cell count and neutrophil count were decreased when compared with those before treatment(P<0.01),and the two values above in the observation group were lower than those in the control group(P<0.05).Scores of lower abdominal pain,menstrual irregularities,aggravation of dysmenorrhea,and distending pain in the breasts in the two groups were decreased when compared with those before treatment(P<0.01),and the four symptom scores in the observation group were lower than those in the control group(P<0.01).Levels of TNF-αand IL-6 in serum in the two groups were decreased when compared with those before treatment(P<0.01),and the two levels in serum in the observation group were lower than those in the control group(P<0.01).Conclusion:In the treatment of sequelae of pelvic inflammatory disease of blood stasis due to qi stagnation type,the therapy of modified Mudan powder combined with antibiotics and Luohua Zizhu tablets can effectively relieve symptoms and inflammation,with significant curative effect.
