窦丹波应用青玉散治疗溃疡性结肠炎的思考和临证体会

Thinking and clinical experience of DOU Dan-bo in treating ulcerative colitis with Qingyu Powder

溃疡性结肠炎(UC)是世界难治性消化疾病,中医药防治临床应用广泛,但多处于辅助地位。窦丹波教授通过长期实践,参考UC容易反复和病理低度炎症始终存在的特点,创造性地指出无论是发作期还是缓解期,湿热、热毒蕴藉肠道始终是核心病机。近年来结合国内外应用经验,发现青黛方药治疗UC疗效显著,并在临床中逐渐形成了团队经验方青玉散。前期小规模单臂实验显示青玉散可有效诱导UC缓解,控制临床症状,促进肠黏膜愈合,提高患者生活质量,并以效案3则展开讨论。希望今后通过高等级循证依据的研究为治疗UC的中药新药开发提供基础。

Ulcerative colitis(UC) is a refractory digestive disease in the world. Traditional Chinese medicine is widely used in clinical prevention and treatment, but most of it is in an auxiliary role. Professor DOU Dan-bo creatively pointed out that dampness-heat, heat-toxin accumulation through intestinal tract is always the core pathogenesis, whether it is in onset or remission, through long-term practice and referring to the characteristics of UC that it is easy to repeat and pathological low-grade inflammation always exists. In recent years, combined with the application experience at home and abroad, it has been found that the herbs of indigo natidae have a remarkable curative effect on UC, and gradually formed the team experience prescription Qingyu Powder in clinical practice. A small-scale single-arm experiment in the early stage showed that Qingyu Powder could effectively induce the remission of UC, control clinical symptoms, promote intestinal mucosa healing, and improve the quality of life of patients. Three effective cases were discussed. It is hoped that the future studies based on high-level evidence-based evidence will provide a basis for the development of new Chinese medicine for the treatment of UC.
