温经通络中药热膏摩联合痛泻药方治疗肝郁脾虚证腹泻型肠易激综合征疗效观察

Curative effect observation of Wenjing Tongluo Hot Ointment combined with Tongxie Yaofang on diarrhea-type irritable bowel syndrome of liver depression and spleen deficiency

目的:观察温经通络中药热膏摩联合痛泻药方治疗肝郁脾虚证腹泻型肠易激综合征(IBS-D)的临床疗效。方法:将96例肝郁脾虚证IBS-D患者随机分为两组,每组48例。治疗组予温经通络中药热膏摩联合痛泻要方治疗,对照组予匹维溴铵片治疗,两组疗程均为4周。观察两组患者治疗前后胃肠道症状评分、肠易激综合征严重程度评分(IBS-SSS)量表、生活质量评价量表(IBS-QOL)评分及中医证候评分,对比IBS-SSS及中医证候疗效,进而判定临床疗效。结果:治疗4周后,两组患者胃肠道症状评分、IBS-SSS评分及腹痛、腹泻、胁肋胀痛、食欲不振、腹胀、肠鸣中医证候评分较治疗前均显著降低(P<0.05);IBS-QOL量表评分均较治疗前升高(P<0.05)。治疗组IBS-SSS总有效率(80.00%)高于对照组(58.14%)(P<0.05),中医证候疗效治疗组总有效率(86.67%)高于对照组(65.12%)(P<0.05)。结论:温经通络中药热膏摩联合痛泻药方治疗肝郁脾虚证IBS-D疗效优于常规匹维溴铵治疗,可以明显减轻胃肠道不良症状,改善患者生活质量,临床疗效显著。

Objective: To observe the clinical effect of Wenjing Tongluo Hot Ointment combined with Tongxie Yaofang on diarrhea-type irritable bowel syndrome(IBS-D) of liver depression and spleen deficiency. Methods: A total of 96 patients with IBS-D of liver stagnation and spleen deficiency were randomly divided into two groups, 48 cases in each group. The treatment group was treated with Wenjing Tongluo Hot Ointment combined with Tongxie Yaofang, and the control group was treated with Pinaverium Bromide Tablets. Both courses of treatment were 4 weeks. Observe the gastrointestinal symptom score, irritable bowel syndrome severity score(IBS-SSS) scale, Quality of Life Evaluation Scale(IBS-QOL) score and TCM syndrome score before and after treatment of the two groups of patients. Compare the efficacy of IBS-SSS and TCM syndrome, then determine the clinical efficacy. Results: After 4 weeks of treatment, the gastrointestinal symptoms score, IBS-SSS score and TCM syndrome score of abdominal pain, diarrhea, costal pain, loss of appetite, abdominal distention and bowel ringing in the two groups were lower than before treatment(P<0.05);IBS-QOL scale scores were higher than before treatment(P<0.05). The total effective rate of IBS-SSS in the treatment group(80.00%) was higher than that in the control group(58.14%)(P<0.05), and the total effective rate in the treatment group(86.67%) of TCM syndromes was higher than that in the control group(65.12%)(P<0.05). Conclusion: Wenjing Tongluo Hot Ointment combined with Tongxie Yaofang is better than conventional Pinaverium Bromide Tablets in the treatment on IBS-D of liver depression and spleen deficiency syndrome.It can significantly reduce gastrointestinal adverse symptoms, improve patients’ quality of life, and has a significant clinical effect.
