基于新冠肺炎疫情前慢性病老年人的健康管理模式,探讨常态化疫情防控背景下依靠社会相关机构和老年人群体共同建设的健康管理模式,以期更好地保证老年人的健康。在常态化疫情防控背景下,针对慢性病老年人群实施健康管理对控制老年人慢性病、提高老年人群抵抗力、降低感染风险有着不可忽视的作用。因此,建设防疫理念下的慢性病健康管理模式,对于助力健康老龄化和新冠肺炎疫情防控工作有着重要意义。

Based on the analysis and discussion of the health management mode of the elderly with chronic diseases before COVID-19 epidemic,the relevant social institutions and the elderly’s groups are relied on to jointly build a health management mode suitable for the background of the normalization of the epidemic prevention and control,in order to better ensure the health of the elderly population.Under the background of the normalization of COVID-19 epidemic prevention and control,the health management on the elderly population with chronic diseases plays important function on the control of the elderly’s chronic disease,improvment of their resistance,and the reduction of infection risks.Therefore the construction of chronic disease health management mode under the concept of epidemic prevention and control plays significant role in the assistance of health aging and COVID-19 epidemic prevention and control.
