不同代偿能力肝硬化患者心理队列的研究与分析

Cohort Study and Analysis of Mental Health Status of Patients With Liver Cirrhosis With Different Compensatory Ability

目的观察代偿期与失代偿期肝硬化患者不同阶段抑郁心理的差异。方法将250例确诊肝硬化患者按《传染病学(第九版)》根据肝组织病理及临床表现分为代偿期组(150例)和失代偿期组(100例)。使用抑郁自评量表(SDS),在患者入院第一天和出院当天调查,对两组肝硬化患者进行心理调查和比较。结果代偿期组和失代偿期组患者的SDS评分在住院期间不同阶段其抑郁状态随着住院时间越长,病情进行加重,其抑郁较入院当天反而加重。结论肝硬化患者的病情严重程度、病程长,反复的病情变化及不良预后直接影响患者的心理状况,应对其进行心理干预,以降低患者的心理负担及压力,提高生存质量,建立对生活的信心。

Objective To observe the difference of depression in different stages of liver cirrhosis patients in compensatory and decompensated liver cirrhosis.Methods According to the"Infectious Diseases(eighth edition)",250 patients with confirmed liver cirrhosis were divided into a compensated group(150 cases)and a decompensated group(100 cases)according to liver histopathology and clinical manifestations.Using the Self-rating depression scale(SDS),the patients were investigated on the first day of admission and the day of discharge,and psychological investigation and comparison of the two groups of patients with liver cirrhosis.Results The SDS scores of the patients in the compensatory and decompensated groups were depressed at different stages during the hospitalization period.As the hospitalization time was longer,their condition worsened,and their depression was worse than that on the day of admission.Conclusion The severity and duration of the disease in patients with liver cirrhosis,repeated changes in the condition,and poor prognosis directly affect the patient’s psychological status.Psychological intervention should be carried out to reduce the patient’s psychological burden and pressure,improve the quality of life,and build confidence in life.
