临床护理路径在AMI患者院前急救中的应用

Application of Clinical Nursing Pathway in Pre Hospital First Aid for Patients with AMI

目的通过建立急性心肌梗死院前急救临床护理路径,应用于院前急救整个过程中,有效利用首次医疗接触后的黄金1 h,缩短院前急救时间,降低心血管疾病发生率及病死率,提高抢救效率。方法收集我院2019年1月至2020年12月院前救治急性心肌梗死的患者122例,按照系统随机抽样的方法,随机分成试验组和对照组。试验组采用急性心肌梗死院前急救临床护理路径进行处理,对照组采用一般急性心肌梗死的院前急救流程处理。比较两组出车反应时间、首份床边心电图绘制时间、院前急救总时间、心血管事件发生率、病死率及患者满意度。结果试验组出车反应时间、首份心电图绘制时间、院前急救总时间、心血管事件发生率及病死率均低于对照组(P<0.05),试验组患者满意度高于对照组(P<0.05)。结论通过急性心肌梗死院前急救临床护理路径的建立和使用,可以缩短院前急救时间,提高抢救效率,降低心血管事件发生率及病死率,提高患者满意度。

Objective To improve the survival rate of patients with acute myocardial infarction by establishing a pre hospital first aid clinical nursing path and making effective use of the golden hour after the first medical contact.Methods One hundred and twenty-two patients with acute myocardial infarction treated before hospital in our hospital from January 2019 to December 2020 were collected.According to the method of systematic random sampling,they were randomly divided into experimental group and control group.The experimental group was treated with the clinical nursing path of pre hospital first aid for acute myocardial infarction,and the control group was treated with general acute myocardial infarction pre-hospital emergency procedures.The vehicle response time,the time to draw the first bedside ECG,the total time of pre-hospital first aid,the incidence of cardiovascular events,the mortality rate and patient satisfaction were compared between the two groups.Results The reaction time of driving,the drawing time of the first ECG,the total time of pre hospital first aid,the incidence of cardiovascular events and mortality in the experimental group were lower than those in the control group(P<0.05),and the patient satisfaction in the experimental group was higher than that in the control group(P<0.05).Conclusion Through the establishment and use of pre hospital first aid clinical nursing path for acute myocardial infarction,we can shorten the pre hospital first aid time,improve the rescue efficiency,reduce the incidence and mortality of cardiovascular events,and improve patient satisfaction.
