抗人球蛋白微柱凝胶法、凝聚胺法在交叉配血中的结果比较 被引量:2

摘要 目的:对比分析抗人球蛋白微柱凝胶法、凝聚胺法在交叉配血试验中的结果。方法:回顾性分析2018年1月~2019年12月在本院行交叉配血试验后输血治疗的327例患者的临床资料,均行抗人球蛋白微柱凝胶法和凝聚胺法两种方法,比较两组的相容性配血阳性率、不规则抗体阳性检测率、各不规则抗体检出情况以及两种方法对不规则抗体检出的敏感度、特异度、准确度情况。结果:在327例输血患者中,微柱凝胶法交叉配血有主测和次测不相合26例,阳性率为7.96%,凝聚胺法交叉配血有主测和次测不相合12例,阳性率为3.67%,微柱凝胶法相容性配血检测的阳性率显著高于凝聚胺法(P<0.05)。微柱凝胶法检测出不规则抗体68例,占比20.80%,凝聚胺法则检测出47例,约14.37%,微柱凝胶法对不规则抗体的阳性检测率明显高于凝聚胺法(P<0.05)。微柱凝胶法抗E检出率高于凝聚胺法(P<0.05),两组的抗D、抗c、抗M、抗C以及自身抗体检测无统计学差异(P>0.05)。微柱凝胶法的敏感度、特异度以及准确度均明显高于凝聚胺法(P<0.05)。结论:微柱凝胶法在交叉配血中,对不规则抗体的检出率较高,其敏感度、特异度以及准确度较高,可作为临床交叉配血试验的首选。 Objective:To comparatively analyze the results of anti-human globulin microcolumn gel technique and manual polybrene test in blood cross matching Methods:The clinical data of 327 patients who received transfusion therapy after cross matching test through anti-human globulin microcolumn gel technique and manual polybrene test in the hospital between January 2018 and December 2019 were analyzed retrospectively.The positive rates of compatibility,positive detection rates of imegular antibodies,detection of each imegular antibody,sensitivities,specificities and accuracy rates of the two methods for detection of imegular antibodies were compared Results:Among the 327 patients undergoing blood transfusion,there were 26 cases with incompatibility of the primary and secondary tests by microcolumn gel technique,and the positive rate was 796%.There were 12 cases with incompatibility of the primary and secondary tests by manual polybrene test,and the positive rate was 3.67% The positive rate of compatibility detected by the microcolumn gel technique was significantly higher than that of the manual polybrene test(P<0.05).68 cases of rregular antibodies were detected by the microcolumn gel technique,accounting for 2080%,and 47 cases were detected by the manual polybrene test,accounting for 1437%.The positive detection rate ofirregular antibodies by the microcolumn gel technique was significantly higher than that of the manual polybrene test(P<0.05).The detection rate of anti-E by the microcolumn gel technique was higher than that by the manual polybrene test(P<0.05).There was no statistical difference in the detection of anti-D,anti-e,anti-M,anti-C or autoantibodies between the two groups(P>0.05).The se nsitivity,specificity and accuracy of the microcolumn gel technique were significantly higher than those of the manual polybrene test(P<0.05).Conclusion:The anti-human g obulin microcolumn gel technique should be the preferred method for clinical cross-matching because of is high detection rate of rregular antibodies,high sensitivity,specificity and accuracy.
