摘要
目的探究网织红细胞相关参数[网织红细胞百分比(RET%)、网织红细胞绝对值(RET#)、网织红细胞血红蛋白(RET-He)]在新生儿ABO溶血病中的诊断价值。方法选取2020年1—10月天津市中心妇产科医院新生儿科收治的确诊为新生儿ABO溶血病患儿66例作为观察组,以同期住院的非ABO溶血患儿51例作为对照组,检测两组患儿静脉血中RET%、RET#、RET-He的水平,并分析RET%、RET#及RET-He三个网织红细胞相关参数对新生儿ABO溶血病诊断的ROC曲线下面积(AUC),同时分别计算RET%、RET#及RET-He的cut-off值。结果观察组的RET%、RET#、RET-He分别为(6.55±2.92)、(0.29±0.17)和(33.71±2.24)pg,均高于对照组的(4.01±3.26)、(0.17±0.08)和(31.12±2.63)pg,差异均有统计学意义(P<0.05);RET%、RET#及RET-He对诊断新生儿ABO溶血病的AUC分别是0.820、0.816、0.776。RET%、RET#及RET-He的cut-off值分别是5.535、0.205、32.500。结论 RET%、RET#以及RET-He是早期诊断新生儿ABO溶血病的重要指标。
Objective To explore the diagnostic value of reticulocyte related parameters [reticulocyte percentage(RET%),reticulocyte absolute value(RET#),reticulocyte hemoglobin(RET-He)] in neonatal ABO hemolytic disease. Methods A total of 66 cases of neonatal ABO hemolytic disease admitted to the Neonatal Department of Tianjin Central Gynecology and Obstetrics Hospital from January to October 2020 were selected as the observation group and 51 children with non-ABO hemolytic disease hospitalized in the same period as the control group. The levels of RET%,RET#,RET-He in venous blood were measured in the two groups. The areas under ROC curve(AUC)of three reticulocyte related parameters of RET%,RET#and RET-He for the diagnosis of neonatal ABO hemolytic disease were analyzed,and the cut-off values of RET%,RET# and RET-He were calculated respectively. At the same time,the cut-off values of RET%,RET# and RET-He were calculated respectively. Results The RET%,RET# and RET-He in the observation group were(6.55±2.92),(0.29±0.17)and(33.71±2.24)pg,higher than(4.01±3.26),(0.17±0.08)and(31.12±2.63)pg in the control group,and the differences were statistically significant(P<0.05).The AUC of RET %,RET# and RET-He in the diagnosis of neonatal ABO hemolysis were 0.820,0.816 and0.776,respectively. The cut-off values of RET%,RET# and RET-He were 5.535,0.205 and 32.500,respectively. Conclusion RET%,RET# and RET-He were the important indexes for early diagnosis of neonatal ABO hemolytic disease.
