血清IMA、Lp-PLA2水平与二尖瓣关闭不全患者心脏瓣膜置换预后的相关性

Correlation between serum IMA,Lp-PLA2 levels and prognosis of cardiovalve replacement in patients with mitral regurgitation

目的分析血清缺血修饰白蛋白(IMA)、脂蛋白相关磷脂酶A2(Lp-PLA2)水平与二尖瓣关闭不全(MR)患者心脏瓣膜置换术(CVR)预后的关系。方法抽取2019年1月至2020年1月开封市中心医院106例MR患者,入院时检测血清IMA、Lp-PLA2水平,完成CVR后,随访1年,评估患者预后结果,比较预后不良、预后良好患者的基线资料和血清IMA、Lp-PLA2水平,分析入院时血清IMA、Lp-PLA2水平与MR患者CVR换预后的关系及入院时血清IMA、Lp-PLA2水平对MR患者CVR预后不良的预测价值。结果本研究中106例MR患者中预后不良患者共22例,占20.75%;预后不良组血清IMA、Lp-PLA2水平高于预后良好组(P<0.05);组间其他资料比较,差异未见统计学意义(P>0.05);经Logistic回归分析结果显示,血清IMA、Lp-PLA2水平与MR患者CVR预后有关,二者高表达可能是MR患者CVR预后不良的风险因子(OR>1,P<0.05)。结论血清IMA、Lp-PLA2高表达与MR患者CVR预后有关。

Objective To analyze the relationship between the serum ischemia modified albumin(IMA)and lipoprotein associated phospholipase A2(Lp-PLA2)levels and the prognosis of cardiovalve replacement(CVR)in patients with mitral regurgitation(MR).Methods A total of 106 patients with MR in Kaifeng Central Hospital from January 2019 to January 2020 were selected,at admission,the serum IMA and Lp-PLA2 levels were detected,the patients were followed up for 1 year after CVR,the prognosis of patients was evaluated,the baseline data and serum IMA and Lp-PLA2 levels in patients with poor prognosis and good prognosis were compared,the relationship between the serum IMA and Lp-PLA2 levels at admission and the prognosis of CVR in MR patients,and the predictive value of the serum IMA and Lp-PLA2 levels at admission for the poor prognosis of CVR in MR patients were analyzed.Results Among the 106 patients with MR in this study,22 patients had poor prognosis,accounting for 20.75%;the serum IMA and Lp-PLA2 levels in poor prognosis group were higher than those in good prognosis group(P<0.05);there was no statistical significant difference in other data between the two groups(P>0.05);through Logistic regression analysis,the results showed that the serum IMA and Lp-PLA2 levels were related to the prognosis of CVR in MR patients,their high expression might be a risk factor for poor prognosis of CVR in MR patients(OR>1,P<0.05).Conclusions The high expression of serum IMA and Lp-PLA2 is related to the prognosis of CVR in MR patients.
