二代动脉导管封堵器介入治疗儿童小型动脉导管未闭的效果

Effect of second-generation arterial duct occluder on children with small patent ductus arteriosus

目的探讨二代动脉导管封堵器(ADOⅡ)介入治疗儿童小型动脉导管未闭(PDA)的效果。方法抽取2018年2月至2020年7月威海市中心医院收治的小型PDA患儿82例,均经影像学和心电图检查确诊,给予ADOⅡ介入治疗。术中术后造影观察封堵器位置、残余分流情况,术后1 d、1个月、3个月复查超声心动图评估封堵效果。结果 82例患儿术中均成功封堵,手术成功率为100%。术后造影显示1例患儿仍有少量残余分流,术后24 h复查分流消失。术后24 h及术后1、3个月行超声心动图检查,结果显示82例患儿PDA均顺利闭合,未见封堵器脱落等并发症。结论 ADOⅡ对儿童小型动脉导管未闭封堵成功率高,术后残余分流较少。

Objective To investigate the effect of the second-generation arterial duct occluder(ADOII)on children with small patent ductus arteriosus(PDA).Methods A total of 82 children with small PDA admitted to Weihai Central Hospital from February 2018 to July 2020 were selected.They were diagnosed by imaging and electrocardiogram and were given ADOⅡinterventional treatment.Intraoperative and postoperative angiography was performed to observe the position of the occluder and the residual shunt.The echocardiogram was reviewed at 1 day,1 month,and 3 months after the operation to evaluate the occlusion effect.Results All 82 children were successfully blocked during the operation,and the operation success rate was 100%.Postoperative angiography showed that there was still a small amount of residual shunt in one child,and the shunt disappeared after 24 hours of reexamination.Echocardiography was performed 24 hours after operation and 1 and 3 months after operation.The results showed that the PDA of 82 children closed smoothly,and there were no complications such as occluder falling off.Conclusions ADOⅡhas a high success rate in occluding small patent ductus arteriosus in children,and there is less residual shunt after operation.
