体素内不相干运动扩散加权成像与T2 mapping评估甲状腺相关性眼病活动性的临床研究 被引量:5

Clinical study of intravoxel incoherent motion diffusion-weighted imaging and T2 mapping in evaluating the activity of thyroid-associated ophthalmopathy

摘要 目的探讨T2 mapping与体素内不相干运动扩散加权成像(intravoxel incoherent motion diffusion-weighted imaging,IVIM-DWI)在甲状腺相关性眼病(thyroid-associated ophthalmopathy,TAO)中的应用价值。材料与方法回顾性分析42名经临床确诊的TAO患者眼眶T2 mapping和IVIM-DWI图像,测量眼外肌T2弛豫时间(T2 relaxation time,T2RT)、表观扩散系数(ADC)、纯扩散系数(D)、灌注相关扩散系数(D^(*))及灌注分数(f)值。比较正常对照组与病例组眼外肌T2RT、ADC、D、D^(*)、f之间的差别,采用受试者工作特征(receiver operating characteristic,ROC)曲线评价其诊断TAO活动性的价值。结果活动期组眼外肌T2RT、D、ADC值明显大于非活动期组(P<0.001)与对照组(P<0.001),非活动期组明显大于对照组(P<0.001);ROC曲线提示T2RT、ADC、D判断TAO活动期的诊断效能均较高,曲线下面积(area under the curve,AUC)>0.5,以D诊断效能最高(AUC=0.882),D与T2RT联合判断TAO活动期的诊断效能进一步提高(AUC=0.893)。结论基于IVIM-DWI和T2 mapping眼外肌定量测量有助于TAO的诊断和分期。
