摘要
目的探究循环转移相关的肺腺癌转录本1(MALAT1)、微小RNA-22(miR-22)表达与老年慢性心力衰竭(CHF)患者心室重构的相关性。方法选取2017年3月至2018年2月于阜新市第二人民医院接受治疗的老年CHF患者142例为研究对象(CHF组),分为Ⅰ~Ⅱ级45例、Ⅲ级49例、Ⅳ级48例;另选取同期健康体检者150例为对照组。收集受试者一般临床资料;患者均行超声心动图检测;Pearson法分析血清MALAT1、miR-22水平与左心室重构(LVR)指标左心室质量指数(LVMI)、左心室射血分数(LVEF)、左心室舒张末期容积(LVEDV)的相关性;多元线性逐步回归模型分析影响CHF患者LVMI水平的相关因素。结果与对照组相比,CHF组心功能Ⅰ~Ⅱ级、Ⅲ级、Ⅳ级血清中MALAT1、LVMI、LVEDV依次升高(P<0.05),miR-22、LVEF依次降低(P<0.05)。Pearson法分析显示,CHF组患者血清MALAT1水平与LVMI、LVEDV呈正相关(P<0.05),与LVEF呈负相关(P<0.05);血清miR-22水平与LVMI、LVEDV均呈负相关(P<0.05),与LVEF呈正相关(P<0.05)。多元线性逐步回归分析显示MALAT1、miR-22(β分别为-5.381和1.632)对LVMI值的影响均有统计学意义(P<0.05)。结论老年CHF患者血清MALAT1水平升高,miR-22水平降低,可能与LVR的发生发展有关,对LVMI值的影响具有统计学意义,可能作为临床诊治老年CHF患者心室重构及病情评估的血清学标志物。
Objective To investigate the correlation between circulating metastasis associated lung adenocarcinoma transcript 1(malat1),microRNA-22(mir-22)expression and ventricular remodeling in elderly patients with chronic heart failure(CHF).Methods 142 elderly CHF patients treated in the second people's Hospital of Fuxin city from March 2017 to February 2018 were selected as the study subjects(CHF group),which were divided into 45 cases of gradeⅠ~Ⅱ,49 cases of grade Ⅲ and 48 cases of grade Ⅳ,and another 150 cases of healthy people's medical examination during the same period were selected as the control group.The general clinical data of the subjects were collected;all patients underwent echocardiography.Pearson's method was used to analyze the correlation between serum levels of malat1 and mir-22 and the left ventricular mass index(LVMI),left ventricular ejection fraction(LVEF),left ventricular end diastolic volume(LVEDV),and multiple linear stepwise regression model was used to analyze the correlation between the levels of LVMI affecting CHF patients factor.Results Malat1,LVMI,LVEDV were sequentially increased(P<0.05)and mir-22,LVEF were sequentially decreased(P<0.05)in serum levelsⅠ~Ⅱ,Ⅲ and Ⅳ of cardiac function in CHF group compared to control group.Pearson's analysis showed that in CHF patients,serum levels of malat1 were positively correlated with LVMI and LVEDV(P<0.05)and negatively correlated with LVEF(P<0.05),and serum levels of mir-22 were negatively correlated with both LVMI and LVEDV(P<0.05)and positively correlated with LVEF(P<0.05).Multiple linear stepwise regression analysis showed that the effects of malat1 and mir-22(-5.381 and 1.632,respectively)on LVMI values were statistically significant(P<0.05).Conclusions The increased level of serum malat1 and decreased level of mir-22 in elderly patients with CHF might be related to the development and progression of LVR,and the effects on LVMI values were statistically significant,which might serve as a serological marker for the clinical diagnosis and evaluation of ventricular remodeling and disease in elderly patients with CHF.
