循证护理对乳头凹陷产妇母乳喂养率和自我效能的影响

Effect of evidence-based nursing on breastfeeding rate and self-efficacy of puerperae with nipple depression

目的探讨循证护理对乳头凹陷产妇母乳喂养率和自我效能的影响。方法选择2019年10月至2020年8月于徐州医科大学附属医院产科分娩的84例乳头凹陷产妇,并随机分为对照组和观察组。实施常规护理的42例产妇设为对照组,在常规护理基础上实施循证护理的42例产妇设为观察组。比较两组产妇的乳头凹陷改善情况、母乳喂养自我效能和母乳喂养率。结果观察组的乳头凹陷总改善率为83.33%,显著高于对照组的47.62%(P<0.05)。入组时,两组间母乳喂养自我效能评分的差异无统计学意义(P>0.05);产后1个月,两组的母乳喂养自我效能评分均显著高于入组时(P值均<0.05),且观察组的评分显著高于对照组(P<0.05)。观察组在产后24 h、1周和1个月的母乳喂养率均显著高于同期对照组(P值均<0.05)。结论针对乳头凹陷产妇实施循证护理可有效改善乳头凹陷,明显提高母乳喂养自我效能和母乳喂养率。

Objective To investigate the effect of evidence-based nursing on breastfeeding rate and self-efficacy of puerperae with nipple depression.Methods 84 cases of puerperae with nipple depression who gave birth from October 2019 to August 2020 at the Affiliated Hospital of Xuzhou Medical University were selected and randomly divided into a control group and an observation group.42 puerperae who received routine nursing were set as the control group,and 42 puerperae who received evidence-based nursing on the basis of routine nursing were set as the observation group.The improvement of nipple depression,breastfeeding self-efficacy and breastfeeding rate were compared between the two groups.Results The improvement rate of nipple depression in the observation group was 83.33%,which was significantly higher than 47.62%in the control group(P<0.05).At the time of entry,there was no significant difference in breastfeeding self-efficacy scores between the two groups(P>0.05).One month after delivery,the breastfeeding self-efficacy scores of the two groups were significantly higher than those when they were enrolled(both P<0.05),and the scores of the observation group were significantly higher than those of the control group(P<0.05).The breastfeeding rate of the observation group at 24 h,1 week and 1 month after delivery was significantly higher than that of the control group during the same period(all P<0.05).Conclusion Evidencebased nursing for puerperae with nipple depression can effectively improve the nipple depression and significantly improve breastfeeding self-efficacy and breastfeeding rate.
