低温等离子射频消融术辅助鼻内镜治疗慢性鼻-鼻窦炎的效果

Effect of low-temperature plasma radiofrequency ablation assisted endoscopic sinus surgery in the treatment of chronic rhinosinusitis

目的分析低温等离子射频消融术辅助鼻内镜治疗慢性鼻-鼻窦炎的效果。方法98例慢性鼻-鼻窦炎患者,随机分为对照组和观察组,每组49例。对照组患者采取鼻内镜手术治疗,观察组采取低温等离子射频消融术辅助鼻内镜手术治疗。比较两组术中鼻腔出血量、术后鼻腔出血量、手术总时间、术后住院时间、术后并发症发生情况以及治疗前后纤毛清除率、纤毛清除速度、鼻腔鼻窦结局测试-20(SNOT-20)评分。结果观察组术中鼻腔出血量、术后鼻腔出血量、手术总时间、术后住院时间分别为(80.21±2.01)ml、(21.21±1.01)ml、(52.21±2.01)min、(6.21±1.21)d,均优于对照组的(99.56±3.35)ml、(30.56±2.91)ml、(83.56±12.21)min、(7.56±2.51)d,差异均具有统计学意义(P<0.05)。观察组的术后并发症发生率2.04%低于对照组的16.33%,差异具有统计学意义(P<0.05)。术后,两组患者的纤毛清除率、纤毛清除速度均较术前有所提高,而SNOT-20评分均较术前降低,且观察组患者的纤毛清除率(78.56±9.21)%、纤毛清除速度(8.56±2.21)mm/min高于对照组的(72.12±3.45)%、(7.21±1.32)mm/min,SNOT-20评分(17.19±1.42)分低于对照组的(23.34±1.68)分,差异均具有统计学意义(P<0.05)。结论低温等离子射频消融术辅助鼻内镜手术对慢性鼻-鼻窦炎的治疗作用确切,可减少出血,减少并发症的发生,加速恢复,改善患者纤毛运动功能和鼻腔功能。

Objective To analyze the effect of low-temperature plasma radiofrequency ablation assisted endoscopic sinus surgery in the treatment of chronic rhinosinusitis.Methods A total of 98 patients with chronic rhinosinusitis were randomly divided into control group and observation group,with 49 cases in each group.Patients in control group were treated with nasal endoscopic surgery,while patients in observation group were treated with low-temperature plasma radiofrequency ablation assisted endoscopic sinus surgery.Both groups were compared in terms of intraoperative nasal blood loss,postoperative nasal blood loss,total operation time,postoperative hospitalization time,postoperative complications,and cilia clearance rate,cilia clearance speed and sino-nasal outcome test-20(SNOT-20)score before and after treatment.Results The intraoperative nasal blood loss,postoperative nasal blood loss,total operation time,and postoperative hospitalization time in the observation group were(80.21±2.01)ml,(21.21±1.01)ml,(52.21±2.01)min,(6.21±1.21)d,respectively,which were all better than(99.56±3.35)ml,(30.56±2.91)ml,(83.56±12.21)min,(7.56±2.51)d in the control group,and the differences were statistically significant(P<0.05).The postoperative complication rate 2.04%of the observation group was lower than 16.33%of the control group,and the difference was statistically significant(P<0.05).After surgery,the cilia clearance rate and cilia clearance speed increased in both groups,while the SNOT-20 score decreased in both groups compared with those before surgery;the cilia clearance rate(78.56±9.21)%and cilia clearance speed(8.56±2.21)mm/min in the observation group were higher than(72.12±3.45)%and(7.21±1.32)mm/min in the control group,and the SNOT-20 score(17.19±1.42)points was lower than(23.34±1.68)points in the control group;all the differences were statistically significant(P<0.05).Conclusion Low-temperature plasma radiofrequency ablation assisted endoscopic sinus surgery has definite therapeutic effect on chronic rhinosinusitis,which can reduce blood loss and complications,accelerate recovery,and improve ciliary motor function and nasal cavity function.
