牙周基础治疗慢性牙周炎患者临床分析

Clinical analysis of basic periodontal therapy for patients with chronic periodontitis

目的探究牙周基础治疗慢性牙周炎患者的临床疗效。方法选取50例慢性牙周炎患者作为实验组,选取同期经口腔检查为健康的50例志愿者作为对照组。对照组予以牙周卫生常识宣讲,实验组进行牙周基础治疗。比较两组临床指标、口腔内有害菌数目、炎性因子水平。结果实验组治疗前牙周袋深度(PD)为(4.12±1.26)mm、出血指数(BI)为(2.38±0.65)、附着丧失(AL)为(3.66±0.43)mm,实验组治疗后PD为(2.46±0.75)mm、BI为(1.60±0.52)、AL为(3.01±0.39)mm,对照组PD为(2.38±0.68)mm、BI为(1.48±0.49)、AL为(2.99±0.37)mm。实验组治疗后及对照组PD、BI、AL水平均低于实验组治疗前,差异有统计学意义(P<0.05)。实验组治疗后及对照组聚合梭杆菌、福赛坦菌、中间普氏菌、牙龈卟啉单胞菌、伴放线菌杆菌数目均少于实验组治疗前,差异有统计学意义(P<0.05)。实验组治疗后及对照组的血清及龈沟液IL-6、IL-8水平均低于实验组治疗前,差异有统计学意义(P<0.05)。结论牙周基础治疗慢性牙周炎患者具有明显的临床疗效,可有效阻止致病菌的生长与繁殖,值得进一步推广应用。

Objective To investigate the clinical efficacy of basic periodontal therapy for patients with chronic periodontitis.Methods 50 patients with chronic periodontitis were selected as the experimental group,and 50 volunteers who were healthy through oral examination were selected as the control group.The control group received a presentation on periodontal hygiene,and the experimental group received basic periodontal therapy.Both groups were compared in terms of clinical indicators,number of harmful bacteria in the oral cavity,and inflammatory factors level.Results In the experimental group,the pocket depth(PD)was(4.12±1.26)mm,the bleeding index(BI)was(2.38±0.65)and the attachment loss(AL)was(3.66±0.43)mm before treatment,and those were(2.46±0.75)mm,BI(1.60±0.52),AL(3.01±0.39)mm after treatment.PD,BI and AL were(2.38±0.68)mm,(1.48±0.49),(2.99±0.37)mm in the control group.The levels of PD,BI and AL in the experimental group after treatment and in the control group were lower than those before treatment in the experimental group,and the differences were statistically significant(P<0.05).The numbers of Fusobacterium nucleatum,Forsatan,Prevotella intermedius,Porphyromonas gingivalis,and Actinomycetes in the experimental group after treatment and in the control group were less than those before treatment in the experimental group,and the differences were statistically significant(P<0.05).The levels of IL-6 and IL-8 in serum and gingival crevicular fluid of the experimental group after treatment and the control group were lower than those of the experimental group before treatment,and the difference was statistically significant(P<0.05).Conclusion The basic periodontal therapy has obvious clinical efficacy for patients with chronic periodontitis,and it can effectively stop the growth and reproduction of pathogenic bacteria,which is worth further promotion and application.
