针刺吕氏对穴治疗颞下颌关节急性前脱位的疗效观察

Efficacy Observation of Acupuncture Lyu′s Acupoint Acute in Treating Anterior Dislocation of Temporomandibular Joint

目的针刺吕氏对穴下关-合谷治疗颞下颌关节急性前脱位的临床观察。方法将120例颞下颌关节急性前脱位的患者随机分为对穴组、合谷组、下关组和常规针刺组,每组30例。各组患者每次30 min治疗,每日1次,每周治疗5 d,2周为1个疗程,连续治疗两个疗程。观察各组患者治疗前后的Fricton颞下颌关节紊乱指数(CMI)、VAS疼痛评分,评定其疗效。结果治疗后,各组的VAS评分、Fricton指数与治疗前相比均下降(P<0.05)。与合谷组相比,其余各组治疗后VAS评分和Fricton指数均明显下降(P<0.01);与对穴组相比,下关组和常规针刺组治疗后VAS评分和Fricton指数均高(P<0.01);下关组与常规针刺组治疗后的VAS评分与Fricton指数差异没有统计学意义(P>0.05)。结论针刺吕氏对穴下关-合谷可有效改善颞下颌关节急性前脱位的临床症状,其效果优于下关组、常规针刺组和合谷组。

Objective:To observe the clinical efficacy of Lyu′s acupuncture at Xiaguan-Hegu in treating acute anterior dislocation of temporomandibular joint(TMJ).Methods:A total of 120 patients with acute anterior dislocation of TMJ were randomly divided into the Lyu′s acupuncture group,Hegu group,Xiaguan group and routine acupuncture group,30 cases in each group.Patients in all groups were treated for 30 min each time,once a day,5 d per week,2 weeks as a course of treatment,for two consecutive courses of.The Fricton TMJ disorder index(CMI)and VAS pain score before and after treatment were observed in the four groups to evaluate the efficacy.Results:After treatment,the VAS score and Fricton index of the 4 groups decreased compared with those before treatment(P<0.05).Compared with the Hegu group,the VAS score and Fricton index of the other three groups were significantly decreased after treatment(P<0.01);compared with the Lyu′s acupuncture group,the VAS score and Fricton index of the Xiaguan group and the routine acupuncture group were higher after treatment(P<0.01);there was no significant difference in the VAS score and Fricton index between the Xiaguan group and the routine acupuncture group after treatment(P>0.05).Conclusion:Acupuncture of Xiaguan-Hegu at Lyu′s acupoint can effectively improve the clinical symptoms of acute anterior dislocation of TMJ,and its effect is better than that of Xiaguan group,routine acupuncture group and Hegu group.
