摘要 目的:探讨CT联合MRI在颅脑动静脉畸形诊断中的应用价值。方法:选择本院2018年1月~2019年12月96例疑似颅脑动静脉畸形患者,均接受CT与MRI检查,最终以动脉造影作为诊断金标准,比较不同检查方法的诊断效果及影像学表现。结果:在96例患者中,最终经动脉造影确诊颅脑动静脉畸形78例,CT检查的灵敏性、特异性、准确性及阳性预测值分别为85.90%、72.22%、83.33%及93.06%,MRI检查的灵敏性、特异性、准确性及阳性预测值分别为89.74%、77.78%、87.50%及94.59%,CT联合MRI检查的灵敏性、特异性、准确性及阳性预测值分别为97.44%、94.44%、96.88%及98.70%,CT联合MRI检查的诊断效能显著高于CT或MRI单独检查(P<0.05)。结论:CT联合MRI检查可清晰显示直接征象及间接征象,提高颅脑动静脉畸形诊断准确率。 Objective:To explore the application value of CT combined with MRI in the diagnosis of craniocerebral arteriovenous malformation.Methods:96 patients with suspected craniocerebral arteriovenous malformations were selected from January 2018 to December 2019 in our hospital.Both received CT and MRI examinations,and finally used arteriography as the gold standard for diagnosis,comparing the diagnostic effects and imaging performance of different examination methods.Results:Among 96 patients,78 cases of craniocerebral arteriovenous malformation were finally confirmed by arteriography.The sensitivity,specificity,accuracy and positive predictive value of CT examination were 85.90%,72.22%,83.33% and 93.06%,respectively.The sensitivity,specificity,accuracy and positive predictive value of MRI examination were 89.74%,77.78%,87.50% and 94.59%,respectively.The sensitivity,specificity,accuracy and positive predictive value of CT combined with MRI were 97.44%,94.44%,96.88% and 98.70%,respectively.The diagnostic efficacy of CT combined with MRI is significantly higher than that of CT or MRI alone(P<0.05).Conclusion:CT combined with MRI can clearly display direct signs and indirect signs,and improve the accuracy of diagnosis of craniocerebral arteriovenous malformation.
