针灸推拿联合康复训练治疗脑卒中后肩手综合征临床研究

Clinical Study on Acupuncture and Moxibustion and Tuina Combined with Rehabilitation Training for Shoulder-Hand Syndrome After Stroke

目的:观察针灸推拿联合康复训练治疗脑卒中后肩手综合征的临床疗效。方法:选取120例脑卒中后肩手综合征患者作为研究对象,使用随机数字法分为观察组与对照组各60例。对照组使用康复训练治疗,观察组使用康复训练联合针灸推拿治疗,比较2组治疗效果。结果:观察组总有效率为95.00%,高于对照组的80.00%(P<0.05)。与同组治疗前比较,2组治疗后视觉模拟评分法(VAS)评分、上肢功能评定表(DASH)评分均降低(P<0.05),Fugl-Meyer运动功能评分量表(FMA)评分、改良Barthel指数(MBI)量表评分均升高(P<0.05);与对照组治疗后比较,观察组治疗后VAS评分、DASH评分较低(P<0.05),FMA评分、MBI评分较高(P<0.05)。结论:针灸推拿联合康复训练治疗脑卒中后肩手综合征疗效显著,可有效减轻患者上肢疼痛情况,恢复其日常生活能力。

Objective: To observe the clinical effect of acupuncture and moxibustion and tuina combined with rehabilitation training for shoulder-hand syndrome after stroke. Methods:A total of 120 cases of patients with shoulder-hand syndrome after stroke were selected as the research objects and randomly divided into the observation group and the control group,60 cases in each group. The control group was treated with rehabilitation training,and the observation group was treated with rehabilitation training combined with acupuncture and moxibustion and tuina. The curative effects in the two groups were compared. Results:The total effective rate was 95.00% in the observation group,higher than that of 80.00% in the control group(P<0.05). After treatment,the scores of Visual Analogue Scale(VAS) and Shoulder,and Hand questionnaire(DASH) in the two groups were decreased when compared with those before treatment(P<0.05),and the scores of FuglMeyer Assessment(FMA) and Modified Barthel Index(MBI) were increased(P<0.05). After treatment,the scores of VAS and DASH in the observation group were lower than those in the control group(P<0.05),and the scores of FMA and MBI were higher(P<0.05). Conclusion:The therapy of acupuncture and moxibustion and tuina combined with rehabilitation training is effective in treating shoulder-hand syndrome after stroke, which can effectively relieve upper limb pain of patients and restore their daily living ability.
