摘要
目的:研究支撑喉镜电视内镜下低温等离子射频消融术治疗会厌囊肿患者的效果.方法:选取2019年1月至2020年10月我院会厌囊肿患者96例,随机分成消融组与切除组(n=48).切除组患者采用传统手术切除囊肿,消融组患者采用支撑喉镜电视内镜下低温等离子射频消融术消除囊肿,观察患者围手术期指标,术前、术后1 d采用放射免疫法测定血管紧张素Ⅱ(AngiotensinⅡ,AngⅡ)、去甲肾上腺素(Norepinephrine,NE)及皮质醇(Cortisol,Cor)水平,同时观察并发症和复发情况.结果:消融组手术时间、术中失血量、住院时间、术后咽痛时间、会厌囊肿评分、NE、Cor、AngⅡ、复发率均明显低于切除组(P<0.05);而两组患者并发症发生率无明显差异(P>0.05).结论:支撑喉镜电视内镜下低温等离子射频消融术治疗会厌囊肿患者,可优化围术期指标,改善症状,降低复发率,减少血清NE、Cor、AngⅡ水平.
Objective:To study the effect of low-temperature plasma radiofrequency ablation under support laryngoscope video endoscopy in the treatment of patients with epiglottic cysts.Methods:A total of 96 patients with epiglottic cysts in our hospital from January 2019 to October 2020 were selected and randomly divided into an ablation group and a resection group(n=48).Patients in the resection group were treated with traditional surgery to remove cysts,and patients in the ablation group were treated with low-temperature plasma radiofrequency ablation under support laryngoscopy and video endoscopy to eliminate cysts.Perioperative indicators were observed.Angiotensin I(Ang I),norepinephrine(NE)and cortisol(Cor)levels were measured by radioimmunoassay before and 1 d after surgery,while complications and recurrence were recorded.Results:The operation time,intraoperative blood loss,hospitalization time,postoperative sore throat time,epiglottic cyst score,NE,Cor,AngI,and recurrence rate in the ablation group were significantly lower than those in the resection group(P<0.05);while there was no significant difference in the incidence of complications between groups(P>0.05).Conclusion:Low-temperature plasma radiofrequency ablation under support laryngoscope and video endoscopy can optimize perioperative indicators,improve symptoms,reduce recurrence rate,and reduce serum NE,Cor,and AngI levels.
