输尿管软镜和超微经皮肾镜碎石术治疗直径1~2 cm肾下盏结石的临床效果比较

Comparison of clinical effects of flexible ureteroscopy and ultramicro percutaneous nephrolithotripsy in the treatment of calculus of lower calyx kidney with diameter of 1~2 cm

目的比较输尿管软镜和超微经皮肾镜碎石术治疗直径1~2 cm肾下盏结石的临床效果。方法将400例直径1~2 cm肾下盏结石患者根据随机数字表法分为对照组(输尿管软镜碎石术)和观察组(超微经皮肾镜碎石术),各200例。比较两组的治疗效果。结果观察组的术中出血量少于对照组,手术时间短于对照组,体外冲击波碎石率低于对照组,结石清除总有效率高于对照组(P<0.05)。术后即刻及术后2 h,观察组血红蛋白水平高于对照组,白细胞计数、降钙素原和C反应蛋白水平低于对照组(P<0.05)。两组患者的NRS评分、住院时间、并发症总发生率无显著差异(P>0.05)。结论与输尿管软镜碎石术相比,超微经皮肾镜碎石术应用在直径1~2 cm肾下盏结石患者治疗中的临床价值更高。

Objective To compare the clinical effects of flexible ureteroscopy and ultramicro percutaneous nephrolithotripsy in the treatment of calculus of lower calyx kidney with diameter of 1 to 2 cm.Methods A total of 400 calculus patients of lower calyx kidney with diameter of 1 to 2 cm were divided into control group(flexible ureteroscopy lithotripsy)and observation group(ultramicro percutaneous nephrolithotripsy)according to random number table method,with 200 cases in each group.The treatment effects of the two groups were compared.Results The amount of intraoperative bleeding in the observation group was less than that in the control group,the operation time was shorter than that in the control group,the rate of extracorporeal shock wave lithotripsy was lower than that in the control group,and the total effective rate of stone removal was higher than that in the control group(P<0.05).Immediately and 2 h after operation,the hemoglobin level in the observation group was higher than that in the control group,and the leukocyte count,procalcitonin and C-reactive protein levels were lower than those in the control group(P<0.05).There were no significant differences in NRS score,hospital stay and total incidence of complications between the two groups(P>0.05).Conclusion Compared with flexible ureteroscopy lithotripsy,ultramicro percutaneous nephrolithotripsy in the treatment of calculus of lower calyx kidney with diameter of 1 to 2 cm has a higher clinical value.
