摘要
目的利用640层容积CT对胰腺癌进行灌注成像,探讨胰腺癌灌注成像的特点,以期丰富胰腺癌灌注的影像数据。方法选取2017年1月至2020年3月因胰腺局限性肿大在本科室行胰腺灌注的104例患者作为研究对象。以胰腺未发现病变的41例患者作为对照组,63例胰腺癌患者作为试验组。对患者的灌注图像进行回顾性分析,利用自带软件(最大斜率法+Patlak法)获得动脉血流量(AF)、等效血容量(Equiv BV)及血流萃取(FE)值。比较两组的时间密度曲线(TDC)及灌注参数(AF、Equiv BV、FE值);记录每位患者计算机自动生成的剂量长度乘积(DLP),并计算有效放射剂量(ED)。结果试验组的TDC呈缓升平坦型或者平坦型,对照组呈速升速降,且试验组的灌注值均低于对照组(P<0.05)。两组灌注扫描过程中所产生的X线ED为(10.99±0.43)mSv。结论640层容积CT灌注成像能够反映胰腺癌灌注减低的特点,辐射剂量低,有较高的临床应用价值,值得推广。
Objective To investigate the perfusion imaging characteristics of pancreatic cancer by using 640 slice volume CT for perfusion imaging of pancreatic cancer,in order to enrich the imaging data of pancreatic cancer perfusion.Methods A total of 104 patients who underwent pancreatic perfusion in the department from January 2017 to March 2020 due to limited pancreatic enlargement were selected as the research objects.Forty-one patients without pancreatic lesions were seted as control group and 63 patients with pancreatic cancer were seted as experimental group.The perfusion images of patients were analyzed retrospectively.The values of arterial blood flow(AF),equivalent blood volume(Equiv BV)and blood flow extraction(FE)were obtained by using the self-contained software(maximum slope method+Patlak method).The time density curve(TDC)and perfusion parameters(AF,Equiv BV and FE value)were compared between the two groups;the dose length product(DLP)automatically generated by computer for each patients were recorded and the effective radiation dose(ED)was calculated.Results The TDC in the experimental group was slowly rising and flat or flat type,and that in the control group was rapidly rising and falling type;the perfusion values in the experimental group were lower than those in the control group(P<0.05).The ED of X-ray produced during perfusion scanning in the two groups was(10.99±0.43)mSv.Conclusion The 640 slice volume CT perfusion imaging can reflect the characteristics of hypoperfusion of pancreatic cancer,with low radiation dose and higher clinical application value,which is worthy of promotion.
