加温湿化高流量鼻导管氧疗治疗慢性阻塞性肺疾病合并夜间睡眠呼吸暂停综合征 被引量:2

摘要 目的:观察分析慢性阻塞性肺疾病(慢阻肺)合并夜间睡眠呼吸暂停综合征患者实施加温湿化高流量鼻导管氧疗的临床效果。方法:选取2019年11月至2020年11月牡丹江市康安医院收治的慢阻肺合并夜间睡眠呼吸暂停综合征患者82例作为研究对象,随机分为对照组和观察组,每组41例。对照组给予低流量氧疗+无创机械通气,观察组给予加温湿化高流量鼻导管氧疗。结果:观察组氧减指数(ODI)、呼吸暂停通气指数(AHI)与COPD症状评估测试(CAT)评分低于对照组,第1秒用力呼气容积(FEV1)与6 min步行距离(6MWD)高于对照组(均P<0.05);观察组并发症少于对照组(P>0.05)。结论:加温湿化高流量鼻导管氧疗效果确切,可显著改善慢阻肺合并夜间睡眠呼吸暂停综合征患者的预后,值得临床推广应用。 Objective:To observe and analyze the clinical effect of hypervolume nasal catheter oxygen therapy with hyperthermia and humidification in patients with chronic obstructive pulmonary disease(COPD)combined with night sleep apnea syndrome.Methods:A total of 82 patients with chronic obstructive pulmonary disease complicated with night sleep apnea syndrome were randomly divided into two groups,with 41 cases in each group.The control group were given low-flow oxygen therapy+non-invasive mechanical ventilation,and the observation group were given high-flow nasal catheter oxygen therapy with hyperthermia and humidification.Results:The Oxygen Desaturation Index(ODI),Apnea Hypopnea Index(AHI)and the COPD Assessment Test(CAT)scores of the observation group were lower than those of the control group,and forced expiratory volume in one second(FEV1)and 6 minute walk distance(6MWD)were higher than those of the control group(Ps<0.05).Complications in the observation group were less than those in the control group(P<0.05).Conclusion:Hyperthermia and hyperflow nasal catheter oxygen therapy is effective and can significantly improve the prognosis of COPD patients with night sleep apnea syndrome.
