脂蛋白关联磷脂酶A_(2)与冠心病及冠状动脉病变严重程度的相关性分析

摘要 目的探讨脂蛋白关联磷脂酶A_(2)(LP-PLA_(2))与冠心病及冠状动脉病变程度的相关性。方法选取2019年1月至2020年12月在江苏省苏北人民医院心内科行冠脉造影确诊冠心病的病患,根据中华医学会心血管病学分会相关指南分成急性冠脉综合征(ACS)组和慢性心肌缺血综合征(CIS)组,并选取造影排除冠心病的患者作为对照组。冠心病患者根据造影结果分为单支病变组、双支病变组、三支病变组,所有患者均抽血行LP-PLA_(2)检测,比较三组LP-PLA_(2)数值,统计LP-PLA_(2)与冠心病病情及传统危险因素的相关性。结果最终纳入139例研究对象,其中ACS组39例,CIS组50例,对照组50例,ACS组年龄、LP-PLA_(2)、低密度脂蛋白胆固醇(L-LDL)、胆固醇(CHO)、脑钠肽(BNP)均高于对照组和CIS组(P<0.05)。三支病变组的LP-PLA_(2)水平[(662.62±102.67)U/L]高于双支病变组[(468.20±70.23)U/L]和单支病变组[(408.81±42.45)U/L],双支病变组LP-PLA_(2)高于单支病变组(P<0.05)。采用Pearson线性相关分析,在冠心病患者中,LP-PLA_(2)与L-LDL、CHO呈正相关性(r=0.562和0.52,P<0.05)。结论脂蛋白关联磷脂酶A_(2)与冠心病具有正相关性,该指标可以预测冠心病病情及冠状动脉病变的严重程度,值得推广应用。 Objective To explore the relationship between lipoprotein-associated phospholipase A_(2)(LP-PLA_(2))and coronary atherosclerotic heart disease and the degree of coronary artery disease.Methods Patients who were hospitalized and had done coronary angiography(CAG)in the department of Cardiology from January 2019 to December 2020,were included in this study.Refer to the relevant guidelines of the Chinese Medical Association Cardiovascular Disease Branch they were divided into acute coronary syndrome(ACS)group and chronic myocardial ischemic syndrome(CIS)group,people who were excluded coronary artery disease by coronary angiography were selected as the control group.According to the CAG results,they were divided into single-vessel disease group,double-vessel disease group,and three-vessel disease group.Blood was drawn from all patients for Lp-PLA_(2)detection,and the Lp-PLA_(2)values of the three groups were compared,and the correlation between Lp-PLA_(2)and coronary heart disease and traditional risk factors was statistically analyzed.Results A total of 139 patients were finally enrolled.Among them,39 cases were in ACS group,50 cases were in CIS group and 50 cases were in control group.In the comparison of age,LP-PLA_(2),low-density lipoprotein cholesterol(L-LDL),cholesterol(CHO),brain natriuretic peptide(BNP),the ACS group is higher than the control group and the CIS group(P<0.05).The level of LP-PLA_(2)in the three-vessel disease group was higher than the double-vessel disease group and single-vessel disease group.The level of LP-PLA_(2)in the double-vessel disease group was higher than that of the single-vessel disease group(P<0.05).Using Pearson linear correlation analysis,LP-PLA_(2)was positively correlated with L-LDL and CHO,and the correlation coefficients were r=0.562 and 0.52,respectively.Conclusion LP-PLA_(2)is independently related with coronary artery disease which can predict coronary heart disease and the severity of coronary artery disease,is worth popularizing and applying.
