二氧化碳冷冻法联合支气管镜下高频电刀、球囊扩张治疗良性气管狭窄的疗效

Efficacy of carbon dioxide cryotherapy combined with bronchoscope high frequency electrotome and balloon dilatation in the treatment of benign tracheal stenosis

摘要 目的探讨二氧化碳冷冻法联合支气管镜下高频电刀、球囊扩张,治疗良性气管狭窄的疗效。方法选取本院2017年3月至2019年3月收治的良性气管狭窄患者82例,回顾性分析患者的临床资料,根据不同治疗方案将其分为两组各41例,对照组采用支气管镜下高频电刀、球囊扩张治疗,观察组采用二氧化碳冷冻法联合支气管镜下高频电刀、球囊扩张治疗,比较两组的疗效、临床症状、肺功能和并发症发生率。结果观察组疗效高于对照组(P<0.05);治疗前观察组的呼吸困难症状评分(2.49±0.62)分、气促症状评分(2.81±0.57)分与对照组的(2.45±0.64)分、(2.74±0.59)分比较无明显差异(P>0.05);治疗后观察组的吸困难症状评分(0.76±0.34)分、气促症状评分(0.85±0.38)低于对照组的(1.09±0.52)分、(1.12±0.46)分(P<0.05);治疗前两组的第1秒用力呼气容积(FEV1)、用力肺活量(FVC)、FEV1/FVC比较无明显差异(P>0.05);治疗后观察组的FVC(2.95±0.28)L、FEV1(2.63±0.25)L、FEV1/FVC(67.24±5.27)%高于对照组的(2.77±0.26)L、(2.47±0.28)L、(64.75±5.46)%(P<0.05);观察组并发症发生率(14.63%)与对照组(19.51%)比较无明显差异(P>0.05)。结论二氧化碳冷冻法联合支气管镜下高频电刀、球囊扩张治疗良性气管狭窄可有效改善患者的临床症状,促进肺功能恢复,疗效显著,安全性可靠。 Objective To investigate the effect of carbon dioxide cryotherapy combined with high frequency electrotome and balloon dilation in the treatment of benign tracheal stenosis. Methods 82 patients with benign tracheal stenosis in our hospital from March 2017 to March 2019 were selected and their clinical data were retrospectively analyzed. According to the different treatment schemes, patients were divided into two groups with 41 cases in each group. The control group was treated with high-frequency electrotome and balloon dilatation under bronchoscope, while the observation group was treated with carbon dioxide cryotherapy combined with high-frequency electrotome and balloon dilatation under bronchoscope. The curative effect, clinical symptoms, pulmonary function, and complication rates in the two groups were evaluated. Results The curative effect of the observation group was higher than that of the control group(P<0.05). Before treatment, there was no significant difference in the dyspnea symptom score and shortness of breath symptom score between the observation group and the control group(2.49±0.62 vs. 2.45±0.64, 2.81±0.57 vs.2.74±0.59, respectively, P>0.05). After treatment, the dyspnea symptom score and shortness of breath symptom score of the observation group was lower than that of the control group(0.76±0.34 vs 1.09±0.52, 0.85±0.38 vs.1.12±0.46, respectively, P<0.05). In addition, before treatment, there was no significant difference in FEV1, FVC, and FEV1/FVC between the two groups(P>0.05), while the FVC, FEV1, and FEV1/FVC of the observation group were higher than that of the control group(2.95±0.28 vs. 2.77±0.26, 2.63±0.25 vs.2.47±0.27, 67.24±5.27 vs. 64.75±5.46, respectively) after treatment. No significant difference was found in complication rates between the observation group(14.63%) and the control group(19.51%)(P>0.05). Conclusion Carbon dioxide cryotherapy combined with bronchoscope high frequency electrotome andballoon dilation in the treatment of benign tracheal stenosis can effectively improve the clinical symptoms of patientsand promotethe recovery of lung function, with significant curative effect and reliable safety.
