β角与VBE、VBE/FVC在评估儿童呼气爆发力方面的一致性分析

摘要 目的评估β角与外推容积(backward extrapolated volume,VBE)、外推容积/用力肺活量(forced vital capacity,FVC)在儿童呼气爆发力方面的一致性。方法选取2018年1月至2020年8月在郑州大学第三附属医院、洛阳市妇幼保健院及漯河市第二人民医院行常规通气肺功能检测的248名学龄期儿童作为研究对象。根据是否患有哮喘,将其分为正常组(n=108)和哮喘组(n=140),比较两组儿童的β角、VBE及VBE/FVC的差异性,采用Kappa检验进行一致性评价,应用Spearman进行相关性分析。结果正常组儿童的β角、VBE及VBE/FVC均显著大于哮喘组,差异有显著统计学意义(P<0.01)。β角与VBE、VBE/FVC的Kappa值分别为0.025及0.009,一致性程度微弱。β角与VBE、VBE/FVC均呈负相关(P<0.01)。结论β角与VBE、VBE/FVC在评估儿童呼气爆发力方面的一致性微弱,目前不推荐直接应用于临床,但可作为辅助参考指标。此外,β角在学龄期儿童哮喘诊断方面具有一定的价值。 Objective To evaluate the consistency of angleβ,backward extrapolated volume(VBE)and VBE/forced vital capacity(FVC)in respiratory explosive power of children.Methods 248 school-aged children who underwent routine ventilated pulmonary function examination in The Third Affiliated Hospital of Zhengzhou University,Luoyang Maternal and Child Health Hospital,Luohe second people’s Hospital from January,2018 to Au-gust,2020 were selected as subjects.According to with or without asthma,the children were divided into normalgroup(n=108)and asthma group(n=140).The differences of angleβ,VBE and VBE/FVC between the twogroups were compared.Kappa test was used to evaluate the consistency,and Spearman was used to analyze the corre-lation of angleβ,VBE and VBE/FVC.Results The angleβ,VBE and VBE/FVC in the normal group were signifi-cantly higher than those in the asthma group(P<0.01).The Kappa values of angleβ,VBE and VBE/FVC were0.025 and 0.009,respectively.Angleβwas negatively correlated with VBE and VBE/FVC(P<0.01).Conclusion Angleβhas little consistency with VBE and VBE/FVC in evaluating children’s expiratory explosive force.Therefore,it was not recommended to be directly used in clinic,but may be used as an auxiliary reference index.Inaddition,the angleβmay have the diagnosis value in school-aged children with asthma.
