慢性阻塞性肺疾病患者TLR4、PD-1、HMGB1水平与肺功能的相关性研究 被引量:10

摘要 目的探讨慢性阻塞性肺疾病(慢阻肺)患者Toll样受体4(TLR4)、程序性死亡受体-1(PD-1)和高迁移率族蛋白B1(HMGB1)与肺功能的相关性。方法选取2016年6月至2018年6月间的110例慢阻肺急性期患者(观察1组)、110例慢阻肺缓解期患者(观察2组)和110例肺功能正常者(对照组)为研究对象,测定血清TLR4、PD-1、HMGB1及肺功能指标第一秒用力呼气容积(FEV1)和第一秒用力呼气容积/用力肺活量(FEV1/FVC)水平,分析血清TLR4、PD-1、HMGB1水平与肺功能指标FEV1、FEV1/FVC的相关性。结果 TLR4、PD-1、HMGB水平相比,观察1组>观察2组>对照组,差异有统计学意义(P<0.05)。FEV1、FEVQ/FVC对比,观察1组<观察2组<对照组,差异有统计学意义(P<0.05)。相关分析结果显示,慢阻肺患者血清TLR4、PD-1、HMGB1水平与FEV1、FEV1/FVC呈显著负相关性(P<0.05)。结论慢阻肺患者肺功能随着TLR4、PD-1和HMGB高表达而呈现出逐渐下降趋势,TLR4、PD-1和HMGB1表达水平上调可能共同参与了慢阻肺的发生及发展。 Objective To investigate the correlation between Toll-like receptor 4(TLR4), programmed death receptor 1(PD-1), and high mobility group protein B1(HMGB1) and lung function in patients with chronic obstructive pulmonary disease(COPD). Methods From June 2016 to June 2018, 110 COPD patients in the acute phase(observation group 1), 110 COPD patients in the remission phase(observation group 2), and 110 patientswith normal lung function(control group) were selected as the research subjects. Serum TLR4, PD-1, HMGB1, andpulmonary function indexes FEV1 and FEV1/FVC were determined. The correlation between serum TLR4, PD-1,HMGB1, and pulmonary function indexes FEV1 and FEV1/FVC was analyzed.Results The levels of TLR4, PD-1,and HMGB were highest in observation group 1, followed by observation group 2 were observed, and lowest in thecontrol group. The difference was statistically significant(P< 0. 05). However, the levels of FEV1 and FEVQ/FVCwere highest in the control group, followed by the observation group 2 were observed, and lowest in observation group1. The difference was statistically significant(P< 0. 05). Correlation analysis results showed that serum TLR4, PD-1, and HMGB1 levels in COPD patients were significantly negatively correlated with FEV1 and FEV1/FVC(P<0. 05).Conclusion The pulmonary function of COPD patients showed a gradually decreasing trend with the highexpression of TLR4, PD-1, and HMGB. The up-regulated expression of TLR4, PD-1, and HMGB1 may participatetogether in the occurrence and development of COPD.
