小青龙汤联合西药治疗哮喘-慢阻肺重叠综合征疗效及对血清炎症因子、肺功能的影响

Curative effect of Xiaoqinglong Decoction combined with western medicine ion asthma-COPD overlap syndrome and its influence on serum inflammatory factors and lung function

目的观察小青龙汤联合西药治疗哮喘-慢阻肺重叠综合征(ACOS)的疗效及对血清炎症因子、肺功能的影响。方法将2018年10月—2019年5月秦皇岛市第三医院呼吸科收治的80例ACOS患者随机分为2组,每组40例。对照组在常规治疗基础上给予布地奈德福莫特罗和噻托溴铵治疗,研究组在对照组治疗基础上加用小青龙汤治疗,2组疗程均为12周。观察2组治疗前后哮喘控制测试(ACT)评分、慢阻肺症状测试(CAT)评分、血清炎症因子[C反应蛋白(CRP)、白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)]和肺功能指标[第1秒用力呼气量(FEV_(1))、FEV_(1)/用力肺活量(FVC)、呼气峰值流速(PEFR)],比较2组临床疗效。结果治疗后2组患者ACT评分及FEV_(1)、FEV_(1)/FVC、PEFR均明显升高(P均<0.05),CAT评分及血清CRP、IL-6、TNF-α水平均明显降低(P均<0.05),且研究组各指标改善情况均明显优于对照组(P均<0.05)。治疗后,研究组总有效率为92.5%(37/40),对照组为75.0%(30/40),2组比较差异有统计学意义(P<0.05)。结论小青龙汤联合西药治疗能有效减轻ACOS患者临床症状,显著改善患者肺功能,并可显著降低炎症因子水平,提高生活质量。

Objective It is to observe the therapeutic effect of Xiaoqinglong Decoction combined with western medicine on asthma-chronic obstructive pulmonary overlap syndrome(ACOS)and its effect on serum inflammatory factors and lung function.Methods Eighty patients with ACOS who were admitted to the Department of Respiratory Diseases of the Third Hospital of Qinhuangdao from October 2018 to May 2019 were randomly divided into 2 groups,with 40 cases in each group.The control group was treated with budesonide formoterol and tiotropium bromide on the basis of conventional therapy,and the study group was treated with Xiaoqinglong Decoction on the basis of therapy in the control group.The treatment course of the two groups was 12 weeks.The asthma control test(ACT)score,chronic obstructive pulmonary symptom test(CAT)score,serum inflammatory factors[C-reactive protein(CRP),interleukin-6(IL-6),tumor necrosis factor-α(TNF-α)]and lung function indicators[forced expiratory volume per second(FEV_(1)),FEV_(1)/forced vital capacity(FVC),peak expiratory flow rate(PEFR)]before and after treatment were observed in the two groups,the clinical efficacy of the two groups was compared.Results After treatment,the ACT scores,FEV_(1),FEV_(1)/FVC,and PEFR of the two groups were significantly increased(all P<0.05),the CAT scores and the levels of serum CRP,IL-6,and TNF-αwere significantly decreased(all P<0.05),and the improvement of these indicators of the study group were significantly better than those of the control group(all P<0.05).After treatment,the total effective rate was 92.5%(37/40)in the study group and 75.0%(30/40)in the control group,the difference between the two groups was statistically significant(P<0.05).Conclusion Xiaoqinglong Decoction combined with western medicine can effectively reduce the clinical symptoms,significantly improve the lung function,and significantly reduce the level of inflammatory factors and improve the quality of life in ACOS patients.
