自制中药热奄包辅助治疗早期膝骨关节炎60例临床观察

Clinical observation on 60 cases of early knee osteoarthritis treated by self-made Chinese materia medica hot compress packet as adjuvant treatment

目的观察自制中药热奄包辅助治疗早期膝骨关节炎的临床疗效。方法将120例早期膝骨关节炎患者采用随机数字表法分为治疗组和对照组,各60例。对照组采用西医常规治疗,治疗组在对照组治疗方法的基础上给予自制中药热奄包外敷。治疗4周后比较2组的临床疗效以及治疗前后的视觉模拟评分法(VAS)评分,西安大略和曼彻斯特大学骨性关节炎指数(WOMAC)评分,膝关节主动屈曲、伸直活动度。结果 2组治疗后VAS、WOMAC评分明显降低,与同组治疗前比较差异均有统计学意义(P<0.05),且治疗组降低更明显,与对照组治疗后比较差异均有统计学意义(P<0.05)。2组治疗后膝关节主动屈曲活动度明显增大,伸直活动度明显减小,与同组治疗前比较差异均有统计学意义(P<0.05),且治疗组改善更明显,与对照组治疗后比较差异均有统计学意义(P<0.05)。对照组总有效率为80.0%(48/60),治疗组为93.3%(56/60),2组比较差异有统计学意义(P<0.05)。结论中药热奄包联合西医常规治疗早期膝骨关节炎临床疗效显著,可明显缓解疼痛,改善关节活动度,值得临床进一步推广应用。

Objective To observe the clinical effect of self-made Chinese materia medica( CMM) hot compress packet in adjuvant treatment of early knee osteoarthritis( KOA). Methods Totally 120 patients with early KOA were divided into treatment group and control group according to random number table method,with 60 cases in each group. The control group was given routine Western medicine treatment,on this basis,the treatment group was externally applied with self-made CMM hot compress packet. After treatment for 4 weeks,the clinical efficacy,visual analogue scale( VAS) score,the Western Ontario and McMaster universities( WOMAC) osteoarthritis index score,the active flexion and extension range of motion( ROM) of knee joint before and after treatment were compared between the 2 groups. Results After treatment,VAS and WOMAC scores of the 2 groups decreased significantly,and the differences were statistically significant compared with those of the same group before treatment( P<0.05),moreover,the decrease was more significant in the treatment group,and the differences were statistically significant compared with those of the control group after treatment( P<0.05). After treatment,the active flexion ROM of the knee joint increased significantly and the extension ROM decreased significantly in the 2 groups,and the differences were statistically significant compared with those of the same group before treatment( P < 0. 05),moreover,the improvement in the treatment group was more significant,and the differences were statistically significant compared with those of the control group after treatment( P<0.05). The total effective rate was 80.0%( 48/60) in the control group and 93.3%( 56/60) in the treatment group,and the difference was statistically significant between the 2 groups( P<0.05). Conclusion It has marked clinical efficacy,can markedly alleviate the pain and improve joint ROM to use CMM hot compress packet combined with routine Western medicine in the treatment of early KOA,which deserves clinical promotion and application.
