探究肿瘤标志物检验用于肺癌诊断的应用价值 被引量:4

摘要 目的研究肿瘤标志物水平检验用于肺癌诊断中的效果。方法方便选取2017年1月—2019年12月该院110例肺癌患者视作肺癌组,另外选取同期110名健康体检者视作健康组,均测定血清CA125、SCCAg、NSE、CEA、CYFRA21-1水平,分析结果。结果肺癌组NSE、CEA、SCCAg、CA125、CYFRA21-1水平分别为(25.23±4.18)ng/ml、(7.68±2.19)ng/mL、(5.82±1.94)ng/mL、(62.38±8.15)U/mL、(10.18±3.23)ng/mL,均高于健康组(10.12±3.35)ng/mL、(1.42±0.62)ng/mL、(0.86±0.34)ng/mL、(21.15±5.19)U/mL、(1.76±0.52)ng/mL,差异有统计学意义(t=29.584、28.846、26.412、44.754、26.993,P<0.05);小细胞肺癌组NSE水平高于其他两组,腺癌组CEA、CA125水平均高于鳞癌、小细胞肺癌组,鳞癌组SCCAg、CYFRA21-1水平均高于其他两组,差异有统计学意义(P<0.05);T3期NSE、CEA、SCCAg、CA125、CYFRA21-1水平均高于T1期、T2期,且T2期均高于T1期,差异有统计学意义(F=4.168、6.319、4.215、8.542、6.862,P<0.05);五项肿瘤标志物联合检测诊断肺癌的灵敏度为80.91%,特异度为86.36%,准确度为83.64%。结论不同临床分期、不同病理类型的肺癌患者NSE、CEA、SCCAg、CA125、CYFRA21-1各肿瘤标志物水平存在差异,联合测定各肿瘤标志物水平能获得较高的诊断效能,辅助临床肺癌的准确诊断。 Objective To study the effect of tumor marker level test in the diagnosis of lung cancer.Methods From January 2017 to December 2019,conveniently select 110 patients with lung cancer in the hospital were regarded as the lung cancer group,and 110 healthy patients in the same period were selected as the healthy group.Serum CA125,SCCAg,NSE,CEA,CYFRA21-1 were measured,analyze the results.Results The levels of NSE,CEA,SCCAg,CA125,and CYFRA21-1 in the lung cancer group were(25.23±4.18)ng/mL,(7.68±2.19)ng/mL,(5.82±1.94)ng/mL,(62.38±8.15)U/mL,(10.18±3.23)ng/mL,all higher than the healthy group(10.12±3.35)ng/mL,(1.42±0.62)ng/mL,(0.86±0.34)ng/mL,(21.15±5.19)U/mL,(1.76±0.52)ng/mL(t=29.584,28.846,26.412,44.754,26.993,P<0.05);NSE levels in the small cell lung cancer group were higher than the other two groups,and the CEA and CA125 levels in the adenocarcinoma group were higher than the squamous cell carcinoma and small cell lung cancer groups,the levels of SCCAg and CYFRA21-1 in the squamous cell carcinoma group were higher than the other two groups,the difference was statistically significant(P<0.05);the levels of NSE,CEA,SCCAg,CA125,and CYFRA21-1 in stage T3 were higher than those in stage T1,T2 stage,and T2 stage was higher than T1 stage,the difference was statistically significant(F=4.168,6.319,4.215,8.542,6.862,P<0.05);the sensitivity of combined detection of five tumor markers in the diagnosis of lung cancer was 80.91%,and the specificity was 86.36%,the accuracy was 83.64%.Conclusion There were differences in the levels of NSE,CEA,SCCAg,CA125,CYFRA21-1 tumor markers in patients with different clinical stages and different pathological types of lung cancer.Combined determination of the levels of tumor markers can obtain higher diagnostic efficiency and assist accurate diagnosis of clinical lung cancer.
