银黄清肺胶囊联合BiPAP对老年AECOPD免疫功能及HNGB1的影响分析

Impact analysis of Yinhuang Qingfei capsules combined with BiPAP on the immune function and HMGB1 for elderly AECOPD patients

目的分析银黄清肺胶囊联合双水平气道正压通气(BiPAP)对老年慢性阻塞性肺疾病急性加重期(AECOPD)患者免疫功能及高迁移率族蛋白B1(HNGB1)水平的影响。方法选择92例老年AECOPD患者作为观察对象,按照随机数字表法分为联合组和BiPAP组,每组46例。BiPAP组在常规药物治疗的同时给予BiPAP治疗,联合组在常规药物治疗的同时给予BiPAP联合银黄清肺胶囊治疗。比较两组临床治疗效果、并发症,以及治疗前后的血清白介素2(IL-2)、γ干扰素(IFN-γ)、肿瘤坏死因子-α(TNF-α)及HNGB1水平变化情况。结果2周治疗完成后,联合组总有效率高于BiPAP组,差异有统计学意义(χ^(2)=4.70,P<0.05)。两组间治疗相关并发症总发生率比较,差异无统计学意义(χ^(2)=0.28,P>0.05)。两组治疗2周后的IL-2水平均较入组时升高,IFN-γ、TNF-α、HNGB1水平均较入组时降低,差异均有统计学意义(t分别=8.52、11.22、2.43、4.14、11.97、5.31、32.28、19.27,P均<0.05),且治疗2周后,联合组IL-2水平高于BiPAP组,IFN-γ、TNF-α、HNGB1水平低于BiPAP组,差异均有统计学意义(t分别=3.65、3.24、7.98、12.29,P均<0.05)。结论银黄清肺胶囊联合BiPAP治疗可提高老年AECOPD患者的临床治疗效果,并在一定程度上改善患者的免疫指标及HNGB1水平。

Objective To analyze the influence of Yinhuang Qingfei capsules combined with biphasic intermittent positive airway pressure(BiPAP)on the immune function and high mobility group box 1(HMGB1)for elderly acute exacerbation of chronic obstructive pulmonary disease(AECOPD)patients. Methods A total of 92 elderly AECOPD patients were collected and randomly divided into the combined group and the BiPAP group,46 cases in each group. The BiPAP group was treated with routine drug therapy and BiPAP,the combined group was treated with routine drug therapy,BiPAP and Yinhuang Qingfei capsules. The clinical effects,treatment complications,and serum interleukin-2(IL-2),interferon-γ(IFN-γ),tumor necrosis factor-α(TNF-α)and HMGB1 levels before and after treatment were compared between the two groups. Results After 2 weeks of treatment,the total effective rate in the combined group was higher that than in the BiPAP group(χ^(2)=4.70,P<0.05). There was no statistically significant difference in the overall treatment complication rate between groups (χ^(2)=0.28,P>0.05).After 2 weeks of treatment,IL-2 levels for two groups were increased,while IFN-γ,TNF-α and HMGB1 levels were declined(t=8.52,11.22,2.43,4.14,11.97,5.31,32.28,19.27,P<0.05),after 2 weeks of treatment,the IL-2 levels in the combined group was higher than that in the BiPAP group,the IFN-γ,TNF-α and HNGB1 levels were lower than those in the BiPAP group(t=3.65,3.24,7.98,12.29,P<0.05).Conclusion Yinhuang Qingfei capsules combined with BiPAP can improve the curative effects,patient’s immune function indicators and HMGB1 levels of elderly AECOPD.
