摘要
目的:研究PICC尖端位置改变与极低出生体质量儿(VLBW)体质量增长的相关性。方法:2019年4月-2020年11月收治VLBW新生儿48例,均实施经外周静脉置入中心静脉导管术(PICC)置管,同时分析VLBW新生儿的220张胸片结果。研究PICC尖端末次位置变化和VLBW体质量增长的相关性。结果:PICC尖端末次位置以3个椎体和2个椎体为主要集中位置;在分析PICC尖端位置发生变化中显示,选择头颈部静脉穿刺,PICC尖端位置变化存在较少的椎体个数;分析PICC尖端位置同VLBW体质量变化发现,患儿采取头颈部静脉穿刺的PICC尖端位置和体质量变化呈现高度的负相关,PICC尖端位置主要为腋静脉、下肢静脉及贵要静脉(P<0.05)。结论:对患儿行PICC不同位置置入,VLBW体质量同PICC尖端位置存在较高关联性。所以,可将其用于判断PICC尖端位置的方式,同时也要根据VLBW身体情况进行其他相关检查。
Objective:To analyze the correlation between PICC tip position change and weight gain in very low birth weight infants.Methods:From April 2019 to November 2020,48 VLBW newborns were selected and PICC catheterization was performed.At the same time,we analyzed the results of 220 chest radiographs of VLBW newborns.We studied the correlation between the last position change of PICC tip and VLBW weight gain.Results:The last position of PICC tip was mainly concentrated in 3 vertebral bodies and 2 vertebral bodies.Displayed in the analysis of changes in PICC tip position,selecting head and neck venipuncture,there were fewer vertebral bodies in the change of PICC tip position.In the analysis of PICC tip position and VLBW weight change,there was a high negative correlation between PICC tip position of head and neck vein puncture in children and weight change,the apical position of PICC was mainly axillary vein,lower limb vein and basilic vein(P<0.05).Conclusion:PICC is placed in different positions,there is a high correlation between VLBW body weight and PICC tip position.Therefore,it can be used to determine the position of PICC tip,at the same time,other relevant examinations shall be carried out according to the physical condition of VLBW.
