瑞芬太尼全身麻醉对剖宫产产妇血流动力学及麻醉质量的影响

Effect of remifentanil general anesthesia on hemodynamics and anesthesia quality of parturients undergoing cesarean section

目的探究瑞芬太尼全身麻醉对剖宫产产妇血流动力学及麻醉质量的影响。方法72例行剖宫产的产妇,均给予全身麻醉,根据麻醉药物的不同分为对照组及观察组,各36例。对照组采用常规麻醉药物,观察组采用瑞芬太尼。对比两组产妇麻醉前后的血流动力学指标[平均动脉压(MAP)、收缩压(SBP)、舒张压(DBP)、心率(HR)],术后苏醒时间和新生儿Apgar评分。结果麻醉后,观察组产妇的MAP、SBP、DBP、HR水平分别为(75.30±13.03)mm Hg(1 mm Hg=0.133 kPa)、(92.24±20.52)mm Hg、(71.67±10.96)mm Hg、(83.63±15.34)次/min,均低于对照组的(92.45±15.86)mm Hg、(116.25±19.41)mm Hg、(82.20±14.97)mm Hg、(114.24±16.12)次/min,差异具有统计学意义(P<0.05)。观察组产妇的术后苏醒时间(5.42±3.24)min短于对照组的(13.54±5.71)min,差异具有统计学意义(P<0.05);两组新生儿出生1、5、10 min的Apgar评分对比,差异无统计学意义(P>0.05)。结论瑞芬太尼应用于剖宫产产妇全身麻醉的效果良好,对产妇血流动力学的影响较小,麻醉诱导平稳,产妇术后可迅速苏醒,值得推广。

Objective To investigate the effect of remifentanil general anesthesia on hemodynamics and anesthesia quality of parturients undergoing cesarean section.Methods A total of 72 cases of parturients undergoing cesarean section all received general anesthesia,and they were divided into control group and observation group according to different anesthetic drugs,with 36 cases in each group.The control group received conventional general anesthesia,and the observation group received remifentanil for anesthesia.The hemodynamic indexes[mean arterial pressure(MAP),systolic blood pressure(SBP),diastolic blood pressure(DBP),heart rate(HR)]before and after anesthesia,postoperative recovery time and neonatal Apgar score were compared between the two groups.Results After anesthesia,the maternal MAP,SBP,DBP,and HR levels in the observation group were(75.30±13.03)mm Hg(1 mm Hg=0.133 kPa),(92.24±20.52)mm Hg,(71.67±10.96)mm Hg,and(83.63±15.34)beats/min,respectively,which were lower than(92.45±15.86)mm Hg,(116.25±19.41)mm Hg,(82.20±14.97)mm Hg,(114.24±16.12)beats/min in the control group,and the differences were statistically significant(P<0.05).The postoperative recovery time in the observation group was(5.42±3.24)min,which was shorter than(13.54±5.71)min in the control group,and the difference was statistically significant(P<0.05).There was no statistically significant difference in neonatal Apgar scores between the two groups at 1,5 and 10 min of birth(P>0.05).Conclusion Remifentanil has a good effect on general anesthesia in parturients undergoing cesarean section,and has little effect on maternal hemodynamics.The induction of anesthesia is stable,and the maternal can wake up quickly after surgery.It is worthy of promotion.
