PICU患儿营养状态与感染病原菌分布及细菌耐药分析 被引量:3

摘要 目的探讨儿童重症监护病房(PICU)患儿营养状态与感染病原菌分布及细菌耐药情况。方法选取68例PICU住院治疗且合并感染患儿作为研究对象,依据《危重症儿童营养评估及支持治疗指南(2018,中国,标准版)》(下称《指南》)评估患儿营养状态并将其分为正常组(22例)、轻度营养不良组(34例)及中-重度营养不良组(12例)。检测三组患儿病原菌感染情况,并进行药敏实验,记录病原菌分布及耐药情况。结果正常组患儿以革兰阳性球菌(占比50.0%)、革兰阴性杆菌(占比45.8%)感染为主,轻度营养不良组和中-重度营养不良组以革兰阴性杆菌(占比58.1%和66.7%)感染为主。三组患儿主要感染革兰阴性杆菌为铜绿假单胞菌、鲍曼不动菌、肺炎克雷伯杆菌。轻度营养不良组和中-重度营养不良组患儿革兰阴性菌细菌耐药率高于正常组患儿,中-重度营养不良组患儿常见菌耐药性更严重。三组患儿主要感染革兰阳性菌为金黄色葡萄球菌、肺炎链球菌。轻度营养不良组和中-重度营养不良组患儿革兰阳性菌细菌耐药率要高于正常组患儿,中-重度营养不良组患儿常见菌耐药性更严重。结论PICU营养不良患儿感染病原菌以革兰阴性杆菌为主,且随着营养状态恶化病原菌耐药性增加。
