脑小血管病总体负担与急性大动脉粥样硬化性脑梗死认知功能障碍的相关性 被引量:10

Association between total burden of CSVD and cognitive impaiment in acute large artery atherosclerotic cerebral infarction patients

摘要 目的探讨脑小血管病(CSVD)总体负担与大动脉粥样硬化性(LAA)脑梗死90 d后认知功能障碍的关系。方法纳入LAA脑梗死患者144例,记录患者的一般资料、入院时美国国立卫生研究院卒中量表(NIHSS)评分等,通过头颅MRI检查评估CSVD总体负担评分。根据发病后90 d是否存在认知功能障碍分为认知功能障碍组和无认知功能障碍组。结果 144例LAA脑梗死患者90 d后认知障碍组66例,认知功能正常组78例,2组年龄、高血压史、受教育年限(<6 a)、入院NIHSS评分比较差异有统计学意义(P<0.05);认知功能障碍组重度CSVD总体负担评分明显高于无认知功能障碍组(P<0.05);CSVD总体负担评分与LAA患者90 d后MoCA评分呈负相关(r=0.61833,P=0.0001)。结论 LAA脑梗死患者CSVD总体负担与其认知功能障碍密切相关。 Objective(CSVD)and cognitive dysfunction after 90 days of cerebral infarction in acute large artery atherosclerotic.MethodsTotally 114 acute large artery atherosclerotic cerebral infarction patients were enrolled,general information,NIHSS score were recorded,and the total burden of CSVD was assessed by MRI findings. According to the presence or absence of cognitive dysfunction 90 days after onset,the patients were divided into cognitive impairment group and non-cognitive impairment group.ResultsThe 144 LAA cerebral infarction patients divided into cognitive impairment group(n=66)and non-cognitive impairment group(n=78). There were significant differences in age,hypertension,years of education(<6 a),NIHSS score between the two groups(P<0.05);the severe CSVD total burden score in the cognitive impairment group was significantly higher than that in the non-cognitive impairment group;the CSVD total burden score was negatively correlated with the MoCA score of LAA cerebral infarction patients(r= 0.61833,P=0.0001).Conclusion ment in LAA cerebral infarction patients.
