6S管理在提高替格瑞洛合理用药水平中的应用

Application of 6S Management in Improving the Level of Rational Use of Ticagrelor

目的:研究6S管理在替格瑞洛用药管理中的应用,以提高替格瑞洛合理用药水平。方法:回顾性分析本院2018年10月~2019年9月(6S管理前)505例和2019年10月~2020年9月(6S管理后)1330例使用替格瑞洛的冠心病患者联合用药和用药依从性数据,并从6S管理的整理、整顿、清洁、规范、素养和安全6个方面分析替格瑞洛的用药问题,制定闭环式替格瑞洛药学服务方案。结果:6S管理后,联合用药品种从人均6.79种降低到人均5.88种,因联合用药引起相互作用的占比从74.9%降低到6.5%,合理用药率从23.8%提升到80.4%,患者用药依从性差的占比从29.7%降低到6.5%,医护人员对替格瑞洛合理用药知识的知晓度从19%提升到91%,患者满意度从90.5%提升至92.5%,有统计学差异(P<0.05)。结论:6S管理可以有效地规范替格瑞洛药学服务流程,保证用药安全,提升患者满意度。

Objective:To study the application of 6S management in the drug administration of ticagrelor in order to improve the level of rational use of ticagrelor.Methods:The combined medication and medication compliance data of 505 patients with coronary heart disease treated with ticagrelor from October 2018 to September 2019(before 6S management)and 1330 patients with coronary heart disese treated with ticagrelor from October 2019 to September 2020(after 6S management)in our hospital were retrospectively analyzed.We analyzed the medication problems of ticagrelor from the 6 aspects of 6S management:sort,straighten,sweep,standardize,sustain and safety,and developed a closed-loop pharmaceutical care plan for ticagrelor.Results:After 6S management,the number of combination drugs decreased from 6.79 per capita to 5.88 per capita,and the proportion of interaction caused by drug combination decreased from 74.9%to 6.5%.The rational drug use rate increased from 23.8%to 80.4%.The proportion of patients with poor medication compliance decreased from 29.7%to 6.5%,the knowledge of rational drug use of ticagrelor among medical staff increased from 19%to 91%,and the satisfaction of patient increased from 90.5%to 92.5%with statistical differences(P<0.05).Conclusion:The 6S management can effectively standardize the pharmaceutical care process of ticagrelor,ensure the medication safety and improve patient satisfaction.
