镜像疗法联合四肢联动训练治疗脑卒中后偏瘫的临床疗效及其对患者下肢功能、日常生活活动能力的影响研究

Clinical Effect of Mirror Image Therapy Combined with Limb Linkage Training on Hemiplegia after Stroke and Its Effect on Lower Limb Function and Activities of Daily Living

背景据统计,脑卒中后偏瘫的发生率高达30%~50%,其可导致患者肢体平衡协调能力及躯干控制能力下降,严重影响患者生存质量。四肢联动训练、镜像疗法可通过不同路径促进偏瘫患者康复,目前尚未见二者联合对脑卒中后偏瘫患者下肢功能、日常生活活动能力影响相关研究。目的分析镜像疗法联合四肢联动训练治疗脑卒中后偏瘫的临床疗效及其对患者下肢功能、日常生活活动能力的影响。方法选取2018年10月至2020年10月首都医科大学附属北京康复医院收治的脑卒中后偏瘫患者100例作为研究对象。采用随机数字表法将患者分为对照组和观察组,各50例。对照组采用四肢联动训练治疗,观察组采用镜像疗法联合四肢联动训练治疗。比较两组患者临床疗效及治疗前后Berg平衡量表(BBS)评分、躯干控制测试(TCT)量表评分、功能性步行量表(FAC)评分、Barthel指数分级。结果观察组患者临床疗效优于对照组(P<0.05)。观察组患者治疗后BBS评分、TCT量表评分高于对照组(P<0.05);对照组、观察组患者治疗后BBS评分、TCT量表评分分别高于本组治疗前(P<0.05)。观察组患者治疗后FAC评分高于对照组(P<0.05);对照组、观察组患者治疗后FAC评分分别高于本组治疗前(P<0.05)。观察组患者治疗后Barthel指数分级优于对照组(P<0.05);对照组、观察组患者治疗后Barthel指数分级分别优于本组治疗前(P<0.05)。结论镜像疗法联合四肢联动训练能有效促进脑卒中后偏瘫患者下肢功能的康复,提高患者平衡能力、躯干控制能力、步行能力及日常生活活动能力,具有较好的推广应用价值。

Background According to statistics,the incidence of hemiplegia after stroke is as high as 30%-50%,which can lead to the decline of limb balance and coordination ability and trunk control ability,and seriously affect the quality of life of patients.Limb linkage training and mirror image therapy can promote the rehabilitation of hemiplegic patients through different paths.At present,there is no relevant study on the effect of the combination of the two on lower limb function and activities of daily living of patients with hemiplegic after stroke.Objective To explore the clinical effect of mirror image therapy combined with limb linkage training on hemiplegia after stroke and its effect on lower limb function and activities of daily living.Methods A total of 100 patients with hemiplegic after stroke treated in Beijing Rehabilitation Hospital,Capital Medical University from October 2018 to October 2020 were selected as the research objects.The patients were divided into control group(50 cases)and observation group(50 cases)according to the random number table method.The control group was treated with limb linkage training,and the observation group was treated with mirror image therapy combined with limb linkage training.The clinical efficacy,Berg Balance Scale(BBS)score,trunk control test(TCT)scale score,Functional Ambulation Category Scale(FAC)score,and Barthel index grading before and after treatment were compared between the two groups.Results The clinical effect of the observation group was better than that of the control group(P<0.05).The scores of BBS and TCT scale after treatment in the observation group were higher than those in the control group(P<0.05).The BBS score and TCT scale score of the control group and the observation group after treatment were higher than those before treatment,respectively(P<0.05).The FAC score after treatment of the observation group was higher than that of the control group(P<0.05).The FAC score of the control group and the observation group after treatment was higher than that before treatment,respectively(P<0.05).The Barthel index grading of the observation group was better than that of the control group after treatment(P<0.05).The Barthel index grading of the control group and the observation group after treatment was better than that before treatment,respectively(P<0.05).Conclusion Mirror image therapy combined with limb linkage training can effectively promote the rehabilitation of lower limb function of patients with hemiplegia after stroke,and improve their balance ability,trunk control ability,walking ability and activities of daily living.It has good popularization and application value.
