3.0T磁共振快速自旋回波序列T_(1)WI及T_(2)FLAIR序列诊断脑转移瘤的价值观察 被引量:2

The value of 3.0T MR CUBE T_(1)WI and CUBE T_(2) FLAIR sequences in the diagnosis of brain metastases

摘要 目的探究3.0 T磁共振快速自旋回波序列(CUBE)T_(1)WI及CUBE T_(2)FLAIR序列诊断脑转移瘤的诊断价值。方法选取2018年5月至2019年12月本院收治的脑转移瘤患者48例,均采用3.0 T磁共振扫描机对其进行扫描,观察两种扫描方式下图像中脑转移病灶的大小、位置、数目及病灶边界,比较两种序列诊断脑转移瘤的诊断价值;测量增强CUBE T_(1)WI及增强CUBE T_(2)FLAIR扫描序列肿瘤及其背景的信号强度(signal indensity,SI),并计算序列上肿瘤与背景对比率(contrast ratio,CR)及对比噪声比(contrast noise ratio,CNR)。结果CUBE T_(1)WI序列发现166个病灶,CUBE T_(2)FLAIR序列发现154个病灶,CUBE T_(1)WI序列的检出率(93.8%)显著高于CUBE T_(2)FLAIR序列(87.0%)(P<0.05),CUBE T_(2)FLAIR的肿瘤/白质、肿瘤/灰质的CR显示均显著高于CUBE T_(1)WI序列诊断(P<0.05),CUBE T_(1)W1的肿瘤/白质、肿瘤/灰质的CNR均显著高于CUBE T_(2)FLAIR诊断(P<0.05)。结论增强CUBE T_(1)WI对脑转移瘤的检出率高于CUBE T_(2)FLAIR序列,CUBE T_(2)FLAIR能更有效地鉴别血管和转移瘤,且能更清晰显示转移灶周围水肿带。 Objective To explore the value of 3.0 T magnetic resonance(MR)CUBE T_(1)W1 and CUBE T_(2)FLAIR sequences in diagnosis of brain metastases.Methods A total of 48 patients with brain metastases who were admitted to the hospital from May 2018 to December 2019 were enrolled.All were scanned with 3.0 T MR scanner.The size,sites,number and boundary of brain metastases lesions by the two scanning methods were observed.The diagnostic value of the two sequences in brain metastases was compared.The signal intensity(SI)of tumors and the background by enhance CUBE T_(1)WI and CUBE T_(2)FLAIR scan sequences was measured.The contrast ratio(CR)and contrast noise ratio(CNR)of tumors and the background on the sequences were calculated.Results There were 166 lesions found in CUBE T_(1)WI sequence,while 154 lesions found in CUBE T_(2)FLAIR sequence.The detection rate of CUBE T_(1)WI sequence was significantly higher than that of CUBE T_(2)FLAIR sequence(93.8%vs 87.0%)(P<0.05).CR values of tumor/white matter and tumor/gray matter in CUBE T_(2)FLAIR sequence were significantly higher than those in CUBE T_(1)WI sequence(P<0.05).CNR values of tumor/white matter and tumor/gray matter in CUBE T_(1)WI were sig-nificantly higher than those in CUBE T_(2)FLAIR(P<0.05).Conclusion The detection rate of brain metastases by en-hanced CUBE T_(1)WI sequence is higher than that by CUBE T_(2)FLAIR sequence.CUBE T_(2)FLAIR can distinguish blood vessels and metastases more effectively,and can more clearly show the edema zone around metastases.
