38例连续性肢端皮炎误诊为甲扁平苔藓患者的研究分析

To analyze 38 cases of acrodermatitis continua of hallopeau misdiagnosed as nail lichen planus

目的对38例连续性肢端皮炎(acrodermatitis continua ofhallopeau,ACH)误诊为甲扁平苔藓(nail lichen planus,NLP)患者进行研究分析。方法选取2017年3月至2020年3月本院38例ACH误诊为NLP患者,针对患者确诊过程及病例结果等对误诊情况作出研究分析。结果患者均通过临床表现被确诊为NLP,采取多种针对NLP的治疗方式后病情依旧反复,后通过查阅相关文献对患者的疾病特征及病理结果进行进一步分析,最终患者被确诊为ACH。治疗2~4周,治疗总有效率为94.7%(36/38);治疗第5~12周,治疗总有效率为97.4%(37/38)。结论ACH是一种较为少见的脓疱性皮肤病,缺少明确的治疗指南,其临床表现与NLP类似,极易出现误诊,因此,在进行确诊时应更加谨慎,避免出现误诊的情况,无法及时为患者进行有效治疗,使患者的病症出现反复或加重的情况。

Objective To study and analyze 38 cases of acrodermatitis continua of Hallopeau(ACH)misdiagnosed as nail lichen planus(NLP).Methods 38 patients with ACH misdiagnosed as NLP in our hospital from March 2017 to March 2020 were selected,and the misdiagnosis situation was analyzed according to the diagnosis process and case results.Results All patients were diagnosed as NLP by clinical manifestations,and the disease was still repeated after taking various treatment methods for NLP.Then,the disease characteristics and pathological results of patients were further analyzed by consulting relevant literature,and finally the patients were diagnosed with ACH.After 2-4 weeks of treatment,the total effective rate of treatment is 94.7%(36/38);after 5-12 weeks of treatment,the total effective rate of treatment is 97.4%(37/38).Conclusion ACH is a relatively rare pustular dermatosis,lack of clear treatment guidelines,its clinical manifestations are similar to NLP,and it is very easy to misdiagnose.Therefore,we should be more careful in the diagnosis of ACh to avoid misdiagnosis,and can not timely carry out effective treatment for patients,so that the patient's symptoms appear repeated or aggravated.
