腰痛型腰椎间盘突出症200例单穴电针治疗的临床观察

Clinical observation on 200 cases of lumbar intervertebral disc herniation with pure lumbago treated by single point electroacupuncture

目的探讨腰痛型腰椎间盘突出症单穴电针治疗的镇痛效果,总结该病疾病诊断和临床治疗的新体会。方法回顾性分析2006年7月至2020年6月本院骨伤科门诊收治的诊断明确的200例单纯腰痛型腰椎间盘突出症患者的临床资料,分析总结其临床表现、单穴电针治疗方法及其镇痛效果,评价患者视觉模拟评分法(VAS)评分及疗效,并辅以典型病例的诊治过程。结果200例腰痛型腰椎间盘突出症患者均采用单穴电针治疗,6次治疗前后,患者的VAS评分变化差异均有统计学意义(P<0.05);痊愈94例,显效52例,有效38例,无效16例,治疗总有效率达为92.0%。结论腰痛型腰椎间盘突出症的突出类型、疼痛机制、临床表现、影像学特征和疾病预后等与典型坐骨神经痛患者均存在较大差异,临床应独立诊断,所推荐的疾病诊断标准对腰痛型腰椎间盘突出症的临床诊断具有指导意义,单穴电针治疗对该病同样有效,从而扩大了该疗法的临床适应证。

Objective To explorethe analgesic effect of single point electroacupuncture therapy in the treatment of lumbar intervertebral disc herniation with pure lumbago(LIDHPL)and to summarize the new experience in the diagnosis and clinical treatment of the disease.Methods The clinical data of 200 patients with LIDHPL treated in the orthopedic and traumatology clinic of our hospital from July 2006 to June 2020 were analyzed retrospectively.The clinical manifestations,single point electroacupuncture treatment and its analgesic effect were analyzed and summarized,and the scores and curative effects of visual analogue scale(VAS)were evaluated,meanwhile supplemented by the diagnosis and treatment of typical cases.Results All patients with LIDHPL were treated with single point electroacupuncture.There was significant difference in VAS score before and after 6 times of treatment(P<0.05);94 cases were cured,52 cases were markedly effective,38 cases were effective and 16 cases were ineffective,the total effective rate was 92.0%.Conclusion The type of lumbar disc herniation,pain mechanism,clinical manifestation,imaging characteristics and disease prognosis of LIDHPL are quite different from those of typical patients with sciatica,therefore,clinical diagnosis should be independent.The recommended diagnostic criteria have guiding significance for the clinical diagnosis of LIDHPL.Single point electroacupuncture therapy is also effective for this special disease,thus,the clinical indications of this therapy are expanded.
