脐部“Ω”形切口微创手术在早产低体质量儿中的应用

Application of“Ω”Shaped Umbilical Incision Minimally Invasive Surgery in Preterm Low Body Weight Infants

目的探讨脐部“Ω”形切口微创手术在早产低体质量儿中的应用价值。方法以2016年7月至2020年7月于郑州大学第三附属医院接受腹部手术的68例早产低体质量儿为研究对象,其中肠闭锁27例,新生儿卵巢囊肿14例,肠旋转不良12例,肥厚性幽门狭窄7例,肠重复畸形8例。回顾性分析患儿临床资料,将接受脐部“Ω”形切口手术的38例患儿纳入观察组,接受传统开腹手术的30例患儿纳入对照组。比较两组手术效果及临床满意度。结果所有病例均顺利完成手术。两组手术时间、术后肠功能恢复时间、住院时间相比,差异无统计学意义(P>0.05)。两组切口感染、切口裂开、切口疝、粘连性肠梗阻发生率相比,差异无统计学意义(P>0.05)。对照组和观察组临床总满意度分别为30.00%(9/30)、94.74%(36/38),观察组总满意度高于对照组(P<0.05)。结论对于无法接受腹腔镜手术的早产低体质量儿,脐部“Ω”形切口手术具有微创及美观的优势,具有一定的应用价值。

Objective To explore the application value of“Ω”shaped umbilical incision minimally invasive surgery in preterm low body weight infants.Methods A total of 68 premature and low-weight infants who underwent abdominal surgery at the Third Affiliated Hospital of Zhengzhou University from July 2016 to July 2020 were included in the study,including 27 cases of intestinal atresia,14 cases of neonatal ovarian cysts,12 cases of intestinal malrotation,7 cases of hypertrophic pyloric stenosis,and 8 cases of intestinal duplication deformity.The clinical data of the children were retrospectively analyzed.Thirty-eight infants who underwent umbilical“Ω”incision surgery were included in observation group,and thirty infants who underwent traditional open surgery were included in control group.The surgical effect and clinical satisfaction of the two groups were compared.Results The operation was successfully completed in all cases.There was no statistical difference between the two groups in operation time,postoperative bowel function recovery time,and hospital stay(P>0.05).There was no statistical difference in the incidence of incisional infection,incision dehiscence,incisional hernia,and adhesive intestinal obstruction between the two groups(P>0.05).The total clinical satisfaction rates of control group and observation group were 30.00%(9/30)and 94.74%(36/38),respectively.The total satisfaction rate of observation group was higher than that of control group(P<0.05).Conclusion For premature infants with low body mass who cannot accept laparoscopic surgery,“Ω”shaped umbilical incision surgery has the advantages of minimal invasiveness and aesthetics,and has certain application value.
