超声乳化联合囊袋内人工晶状体植入术治疗老年糖尿病性白内障患者的效果 被引量:2

Effect of Phacoemulsification Combined with Intracapsular Intraocular Lens Implantation on Elderly Patients with Diabetic Cataract

摘要 目的分析超声乳化联合囊袋内人工晶状体(IOL)植入术治疗老年糖尿病性白内障(DC)患者的效果。方法选取2019年6月至2020年12月开封市中心医院收治的74例年龄>60岁的DC患者,根据手术方法分为两组。常规组(37例)接受白内障囊外摘除联合IOL植入术,超声组(37例)接受超声乳化联合囊袋内IOL植入术。比较两组术前、术后3个月视功能(最佳矫正近视力、远视力及非矫正近视力、远视力)、氧化应激水平[血清超氧化物歧化酶(SOD)、谷胱甘肽过氧化物酶(GSH-Px)、丙二醇(MDA)]以及生活质量[中文版低视力者生活质量量表(CLVQOL)评分]。结果与常规组比较,术后3个月超声组非矫正近视力、远视力及血清GSH-Px、SOD水平均较高,MDA水平较低(P<0.05);术后3个月超声组CLVQOL评分较常规组高(P<0.05)。结论超声乳化联合囊袋内IOL植入术治疗老年DC患者有利于减轻氧化应激反应,提高视功能,改善生活质量。 Objective To analyze the effect of phacoemulsification combined with intracapsular intraocular lens(IOL)implantation in the capsular bag in the treatment of elderly patients with diabetic cataract(DC).Methods A total of 74 patients with DC aged over 60 years old in Kaifeng Central Hospital from June 2019 to December 2020 were selected and divided into two groups according to the surgical method,with 37 cases in each group.Conventional group received extracapsular cataract extraction combined with IOL implantation,and ultrasound group received phacoemulsification combined with intracapsular IOL implantation.The visual function(best corrected near vision,best corrected distance vision,uncorrected near vision and uncorrected distance vision),oxidative stress levels[serum superoxide dismutase(SOD),glutathion peroxidase(GSH-Px)and malondialdehyde(MDA)]and quality of life[Chinese version low vision quality of life questionnaire(CLVQOL)scores]before and 3 months after surgery were compared in the two groups.Results Three months after surgery,the uncorrected near vision,uncorrected far vision,serum GSH-Px level and SOD level in ultrasound group were higher than those in conventional group,and the MDA level in ultrasound group was lower than that in conventional group(P<0.05).Three months after surgery,the CLVQOL score in ultrasound group was higher than that in conventional group(P<0.05).Conclusion The application of phacoemulsification combined with intracapsular IOL implantation in the treatment of patients with DC aged over 60 years old is beneficial to reduce oxidative stress,improve visual function and improve the quality of life.
