改良青龙摆尾针法联合膝关节旋转屈伸手法对膝骨关节炎的疗效

Therapeutic Effect of Modified Qinglong Tail-Waving Needle Method Combined with Knee Joint Rotation Flexion and Extension Maneuver on Knee Osteoarthritis

目的探讨改良青龙摆尾针法联合膝关节旋转屈伸手法对膝骨关节炎(KOA)的疗效。方法选取2018年4月至2021年4月就诊于平顶山市第二人民医院的86例KOA患者,采用随机数表法分为观察组(43例)与对照组(43例)。两组均接受常规治疗,对照组在此基础上接受膝关节旋转屈伸手法治疗,观察组在对照组基础上联合改良青龙摆尾针法治疗。比较两组治疗效果、疼痛程度、中医证候评分、炎症因子水平、膝关节相关指标。结果观察组治疗总有效率高于对照组(90.70%比74.42%,P<0.05)。治疗后观察组血清基质金属蛋白酶-3(MMP-3)、基质金属蛋白酶-9(MMP-9)、基质细胞衍生因子-1(SDF-1)、白介素-1β(IL-1β)、白介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)水平低于对照组(P<0.05),滑膜组织厚度(STT)、髌上囊积液深度小于对照组(P<0.05),疼痛、中医证候评分低于对照组(P<0.05)。结论改良青龙摆尾针法联合膝关节旋转屈伸手法治疗KOA效果较好,可有效减轻炎症和疼痛,缓解症状,改善膝关节功能。

Objective To explore the effect of modified Qinglong tail-waving needle method combined with knee joint rotation flexion and extension manipulation on knee osteoarthritis(KOA).Methods A total of 86 patients with KOA who were admitted to Pingdingshan Second People’s Hospital from April 2018 to April 2021 were selected and divided into observation group(43 cases)and control group(43 cases)by random number table method.Both groups received conventional treatment,control group received knee joint rotation flexion and extension therapy on this basis,and observation group was combined with modified Qinglong tail-waving acupuncture therapy on the basis of control group.The treatment effect,pain degree,TCM syndrome score,inflammatory factor level,and knee joint related indexes were compared between the two groups.Results The total effective rate of treatment in observation group was higher than that in control group(90.70%vs.74.42%,P<0.05).After the treatment,in observation group,serum matrix metalloproteinase-3(MMP-3),matrix metalloproteinase-9(MMP-9),stromal-derived factor-1(SDF-1),interleukin-1β(IL-1β),interleukin-6(IL-6),and tumor necrosis factor-α(TNF-α)levels were lower than those in control group(P<0.05),the synovial tissue thickness(STT)and the depth of suprapatellar sac effusion were lower than those in control group(P<0.05),and the scores of pain and TCM syndromes were lower than those in control group(P<0.05).Conclusion The modified Qinglong tail-waving needle method combined with the knee joint rotation flexion and extension method is effective in the treatment of KOA,which could effectively reduce inflammation and pain,relieve symptoms,and improve knee joint function.
