显微神经外科手术治疗三叉神经痛的临床疗效观察

Observation on the clinical effect of microneurosurgery on trigeminal neuralgia

目的分析三叉神经痛在临床治疗中使用显微神经外科手术治疗的效果。方法72例三叉神经痛患者,以随机数字表法分为对照组及观察组,每组36例。对照组接受三叉神经感觉根切断术治疗,观察组接受显微神经外科手术治疗。对比两组患者的疼痛缓解率、治疗优良率、症状缓解时间、匹兹堡睡眠质量指数表(PSQI)评分。结果观察组疼痛缓解率97.22%高于对照组的80.56%,差异具有统计学意义(P<0.05)。观察组治疗优良率97.22%高于对照组的75.00%,差异具有统计学意义(P<0.05)。观察组面部抽搐缓解时间(5.13±1.13)d、流涎缓解时间(2.20±0.59)d、流泪缓解时间(6.11±1.35)d、面部疼痛缓解时间(1.98±0.41)d短于对照组的(7.79±1.36)、(4.36±0.79)、(7.98±1.52)、(3.51±0.69)d,差异具有统计学意义(P<0.05)。观察组的睡眠质量、入睡时间、睡眠时间、睡眠效率、睡眠障碍、日间功能、安眠药物评分及总分均低于对照组,差异具有统计学意义(P<0.05)。结论三叉神经痛在临床治疗中使用显微神经外科手术治疗的疗效突出,具有临床推广价值。

Objective To analyze the effect of microneurosurgery on trigeminal neuralgia.Methods A total of 72 patients with trigeminal neuralgia were divided into the control group and the observation group by random numerical table,with 36 cases in each group.The control group received trigeminal sensory root rhizotomy,and the observation group received microneurosurgery.Both groups were compared in terms of pain remission rate,excellent and good rate of treatment,symptom remission time,and Pittsburgh Sleep Quality Index(PSQI)scores.Results The pain remission rate 97.22%of the observation group was higher than 80.56%of the control group,and the difference was statistically significant(P<0.05).The excellent and good rate of treatment 97.22%in the observation group was higher than 75.00%in the control group,and the difference was statistically significant(P<0.05).The facial twitch remission time(5.13±1.13)d,salivation remission time(2.20±0.59)d,lacrimation remission time(6.11±1.35)d,and facial pain remission time(1.98±0.41)d in the observation group were shorter than(7.79±1.36),(4.36±0.79),(7.98±1.52),(3.51±0.69)d in the control group,and the difference was statistically significant(P<0.05).The sleep quality,time to fall asleep,sleep duration,sleep efficiency,sleep disturbance,daytime function,sleeping drug score and total score in the observation group were lower than those of the control group,and the difference was statistically significant(P<0.05).Conclusion Microneurosurgery has outstanding efficacy in the clinical treatment of trigeminal neuralgia,and it has the value of clinical promotion.
