显微夹闭术与介入栓塞术治疗颅内破裂动脉瘤的临床比较

Clinical Comparison of microsurgical Clipping and interventional Embolization in the Treatment of ruptured intracranial Aneurysms

目的探讨显微夹闭术与介入栓塞术治疗颅内破裂动脉瘤的临床效果。方法选择2016年7月至2019年7月淅川县人民医院收治的96例颅内破裂动脉瘤患者,按随机对照原则分为两组,各48例。显微组给予显微夹闭术治疗,介入组给予介入栓塞治疗。比较两组患者手术一般指标(手术时间、住院时间、术中出血量)、近期预后效果及围术期并发症。结果显微组手术时间、术后住院时间、术中出血量大于介入组,差异有统计学意义(P<0.05);显微组优良率略低于介入组,无统计学意义(P>0.05);显微组并发症发生率高于介入组,差异有统计学意义(P<0.05)。结论显微夹闭术及介入栓塞术均能有效治疗颅内破裂动脉瘤患者,但介入栓塞术在术后恢复效果更优于显微夹闭术,安全性高,并发症风险低。

Objective To explore the clinical effect of microsurgical clipping and interventional embolization in the treatment of ruptured intracranial aneurysms.Methods From July 2016 to July 2019,96 patients with ruptured intracranial aneurysms admitted to Xichuan county people’s hospital were randomly divided into two groups(48 cases each).The microsurgical group was treated with microsurgical clipping,and the interventional group was treated with interventional embolization.The time of operation,hospital stay and intraoperative hemorrhage,short-term prognosis and perioperative complications were compared between the two groups.Results The operation time,postoperative hospitalization time and blood loss in microsurgery group were significantly longer than those in intervention group(P<0.05),and the excellent and good rate in microsurgery group was slightly lower than that in intervention group(P>0.05);the complication rate in the microsurgery group was higher than that in the interventional group(P<0.05).Conclusion Both microsurgical clipping and interventional embolization are effective in the treatment of ruptured intracranial aneurysms,but the effect of interventional embolization is better than microsurgical clipping,the safety is high and the risk of complications is low.
