中药口服联合穴位敷贴治疗肺癌术后痰湿蕴肺型咳嗽的临床观察

Clinical observation of oral administration of traditional chinese medicine combined with acupoint application in the treatment of postoperative cough of phlegm-dampness accumulation of lung in patients with lung cancer

目的观察小青龙汤合四君子汤加减联合穴位敷贴治疗非小细胞肺癌术后痰湿蕴肺型咳嗽的临床疗效。方法将72例非小细胞肺癌术后痰湿蕴肺型咳嗽的患者随机分为治疗组和对照组,每组36例。治疗组予小青龙汤合四君子汤加减,配合穴位敷贴,对照组予西药盐酸氨溴索片口服,各治疗2周。治疗前后对比咳嗽视觉模拟评分、中医临床症候、细胞因子、安全性等指标,并记录止咳起效时间。结果治疗组总有效率91.43%,高于对照组71.43%,差异有统计学意义(P<0.05)。治疗后,治疗组咳嗽视觉模拟评分下降(3.54±1.92)分,对照组下降(2.17±1.74)分,差异有统计学意义(P<0.05)。治疗组的止咳起效时间为(3.48±1.36)d,较对照组明显缩短(P<0.05)。在中医临床症候评分的总分、咳嗽、咯痰方面,治疗组优于对照组(P<0.05)。两组治疗前后白介素2受体(interleukin 2 receptor,IL-2R)、IL-6及IL-8水平比较,差异无统计学意义(P>0.05)。两组均未见明显不良反应。结论小青龙汤合四君子汤加减联合穴位敷贴治疗肺癌术后痰湿蕴肺型咳嗽有效,相较对照组,可以明显缩短止咳起效时间,改善咳嗽症状,且安全性良好,可以补充临床上肺癌术后咳嗽治疗方案的欠缺。

Objective To observe the clinical efficacy of Xiaoqinglong decoction combined with Sijunzi decoction combined with acupoint application in the treatment of postoperative cough with phlegm-dampness accumulation of lung in patients with non-small cell lung cancer.Methods A total of 72 patients of postoperative cough with phlegm-dampness accumulation of lung in patients with non-small cell lung cancer were randomly divided into treatment group and control group,with 36 cases in each group.The treatment group was treated with Xiaoqinglong decoction and Sijunzi decoction,combined with external acupoint application.The control group was given ambroxol hydrochloride tablets.Each treatment is 2 weeks.The visual analogue score of cough,traditional Chinese medicine(TCM) syndrome score scale,changes of cytokines levels and safety evaluation were evaluated before and after the treatment.And the effective time of relieving cough was recorded.Results The total effective rate was 91.43% in the treatment group and 71.43% in the control group(P<0.05).After the treatment,the visual analogue score of cough decreased(3.54±1.92) points in the treatment group and(2.17±1.74) points in the control group(P<0.05).The effective time of relieving cough in the treatment group was (3.48±1.36) days,which was significantly shorter than that in the control group(P<0.05).TCM syndrome score scale,in total score,cough and expectoration,the treatment group was better than the control group(P<0.05).There were no significant differences in the level of interleukin 2 receptor(IL-2R),IL-6 and IL-8 between the two groups before and after treatment(P>0.05).There were no obvious adverse reactions in both groups.Conclusion Xiaoqinglong decoction combined with Sijunzi decoction combined with acupoint application is effective in treating postoperative cough of phlegm-dampness accumulation of lung in patients with lung cancer.Compared with the control group,it can significantly shorten the onset time of relieving cough,improve cough symptoms,and has good safety.And can supplement the lack of clinical treatment of postoperative cough in lung cancer.
