免疫球蛋白A、MeCP2和炎症因子水平与特发性肺纤维化患者肺功能的相关性

Correlation between the levels of immunoglobulin A,MeCP2 and inflammatory factors and pulmonary function in patients with idiopathic pulmonary fibrosis

目的探讨免疫球蛋白A(IgA)、甲基化CpG结合蛋白(MeCP2)和炎症因子水平与特发性肺纤维化患者肺功能的相关性。方法检测并比较特发性肺纤维化患者(观察组)与健康者(健康对照组)的IgA、MeCP2及炎症因子TNF-α、IL-6水平,记录2组的用力肺活量(FVC)、第一秒用力呼吸量(FEV_(1))、呼气流量峰值(PEF)以及6 min步行距离。结果观察组与健康对照组体质量指数、饮酒时间、吸烟时间及高血压、2型糖尿病比例比较,差异均有统计学意义(P<0.05)。观察组血清IgA、MeCP2、TNF-α和IL-6水平均高于健康对照组,FVC、FEV_(1)、PEF及6 min步行距离均低于健康对照组,差异均有统计学意义(P<0.05)。观察组血清IgA、MeCP2、TNF-α和IL-6水平分别与FVC、FEV_(1)、PEF及6 min步行距离呈负相关(P<0.05),IgA、MeCP2、TNF-α和IL-6联合检测对特发性肺纤维化诊断的AUC值为0.833。结论血清IgA、MeCP2、TNF-α和IL-6水平与特发性肺纤维化患者的肺功能相关,4个指标联合对于特发性肺纤维化具有一定的诊断价值。

Objective To investigate the correlation between the levels of IgA,MeCP2 and inflammatory factors and pulmonary function in patients with idiopathic pulmonary fibrosis.Methods The levels of IgA,MeCP2,TNF-α and IL-6 were detected and compared between patients with idiopathic pulmonary fibrosis(observation group)and healthy subjects(healthy control group).The FVC,FEV_(1),PEF and 6-minute walking distance were recorded.Results There were significant differences between the observation group and the healthy control group in terms of body mass index,drinking history,smoking history,hypertension,and type 2 diabetes(P<0.05).Serum levels of IgA,MeCP2,TNF-αand IL-6 in the observation group were higher than those in the healthy control group(P<0.05).FVC,FEV_(1),PEF and 6-minute walking distance in the observation group were lower than those in the healthy control group(P<0.05).Serum levels of IgA,MeCP2,TNF-α and IL-6 in the observation group were negatively correlated with FVC,FEV_(1),PEF and 6-minute walking distance respectively(P<0.05).The AUC of the combined detection of IgA,MeCP2,TNF-α and IL-6 for the diagnosis of idiopathic pulmonary fibrosis was 0.833.Conclusions Serum levels of IgA,MeCP2,TNF-αand IL-6 are correlated with the lung function in patients with idiopathic pulmonary fibrosis.The combined detection of these four indicators has certain diagnostic value for idiopathic pulmonary fibrosis.
