摘要
目的了解广州市健康人群麻疹抗体水平。方法在广州市抽取健康人群1 408人,调查免疫史等信息,采集血清样本检测麻疹免疫球蛋白G(Immunoglobin G,IgG)抗体,分析抗体阳性率和中位浓度。结果调查对象麻疹IgG抗体总阳性率为89.99%,中位浓度为929.4 mIU/ml。<1岁和10~<15岁抗体阳性率为78.40%和80.92%,2~<20岁调查对象的麻疹IgG抗体浓度与年龄之间呈负相关(r=-0.214,P<0.01),接种0、1、≥2剂次含麻成分疫苗(measles-containing vaccine, MCV)人群的抗体阳性率分别为57.14%、88.57%和91.39%(χ^(2)=50.841,P<0.01),末次疫苗接种后0~<5、5~<10和≥10年的抗体阳性率和抗体浓度总体呈下降趋势(H=15.487,P=0.000)。结论广州市儿童和青少年群体抗体水平随年龄增长而下降;免疫后抗体水平随末次疫苗接种后时间的推移而衰减。建议加强对大龄儿童和青少年群体抗体水平监测。
Objective To understand the seroprevalence of measles among healthy population in Guangzhou in 2018. Methods A total of 1 408 healthy individuals were randomly recruited in the survey in Guangzhou in 2018.Blood samples were collected for the detection of serum IgG antibody against measles using ELISA.Meanwhile, the information of measles immunization history was acquired in the survey. Results The overall seropositivity rate was 89.99% with a geometric mean concentration(GMC) of IgG antibody of 929.4 mIU/mL.The positive rate was 78.40% in the age group of <1 and was 80.92% in the age group of 10-<15.The IgG antibody level was negatively correlated with age among individuals aged 2-<20 years(r=-0.214, P<0.01).The antibody positive rates were 57.14%, 88.57% and 91.39% among subjects who received 0 dose, 1 dose and ≥2 doses of measles-containing vaccine(MCV)(χ^(2)=50.841, P<0.01).Both the antibody positive rate and GMC showed down-ward trends within 5 years, 5-<10 years and over 10 years after the last dose of MCV vaccination(H=15.487, P=0.000). Conclusions Measles antibody level decreases with age and over years after the last dose of MCV vaccination in children and adolescents.It is suggested to strengthen the monitor of antibody level in bigger kids and adolescents.
