德宏州艾滋病抗病毒治疗质量控制管理技术推广与应用

Popularization and application of quality control and management technologies for AIDS antiretroviral therapy in Dehong Prefecture

目的采用质量评估管理技术对德宏州艾滋病抗病毒治疗工作进行量化评估和持续改进。方法按照《云南省艾滋病抗病毒治疗质量控制手册》相关要求,结合德宏州实际工作情况,对德宏州各级65个治疗点艾滋病抗病毒治疗质量控制中的核心指标进行评估,同时实施推广云南省艾滋病抗病毒治疗量化评价管理技术。结果确认德宏州艾滋病抗病毒治疗量化评价内容及评分标准,完成全州质量评估。对比开展艾滋病抗病毒治疗质量控制前、后全州病毒载量(viral load, VL)检测比例(χ^(2)=1 966.078,P<0.01)、病毒抑制率(χ^(2)=67.399,P<0.01)、治疗成功率(χ^(2)=173.581,P<0.01)均有所提高,当年病死率(χ^(2)=42.497,P<0.01)降低。结论德宏州艾滋病抗病毒治疗质量控制管理技术应用实现了对艾滋病抗病毒治疗各环节工作的量化评估,各级管理部门落实横向及纵向实时管理,有助于提高抗病毒治疗质量。

Objective To quantitatively evaluate and constantly improve the AIDS antiretroviral therapy(ART) work in Dehong Prefecture by quality assessment management technology.Methods The core indexes of quality control of AIDS antiretroviral therapy in Dehong Prefecture were evaluated in combination with actual service conditions of Dehong Prefecture based on relevant requirements of Quality Control Manual of Antiretroviral Therapy in Yunnan Province,and the quantitative evaluation management technologies for AIDS antiretroviral therapy in Yunnan was promoted.Results The quantitative evaluation contents and criterion for evaluation of AIDS antiretroviral therapy in Dehong Prefecture were confirmed, and overall quality evaluation of Dehong Prefecture was completed.The detection proportion of viral load(VL)(χ^(2)=1 966.078,P<0.01),virus inhibition rate(χ^(2)=67.399,P<0.01) and success rate of therapy(χ^(2)=173.581,P<0.01) increased before and after quality control of AIDS antiretroviral therapy contrastively implemented, and the mortality rate for current year(χ^(2)=42.497,P<0.01) decreased.Conclusion The application of quality control and management technologies for AIDS antiretroviral therapy in Dehong Prefecture realizes the quantitative evaluation on all aspects of AIDS antiretroviral therapy, and the management departments at all levels implement horizontal and vertical real-time management, making for improving the quality of antiretroviral therapy.
