深圳市宝安区中心医院“两融合一协同”的改革实践

Reform and practice of“Two Integration,One Collaboration”model in Bao’an Central Hospital of Shenzhen City

卫生服务整合和医防融合是我国当前深化医药卫生体制改革的重要方向和着力点。深圳市宝安区中心医院集团积极探索紧密型基层医疗服务联合体的管理体制和运行机制,打造医院与社康机构融合发展、医疗与预防融合发展、全科与专科协同服务(即“两融合一协同”)模式,实现了慢性病和老年患者全专闭环健康管理、社康机构服务能力和服务水平双提升的局面,受到了辖区居民和专业同行的广泛认可。文章对两融合一协同模式的主要举措、实践效果及主要经验进行阐述,以为全国基层医疗卫生机构的医防融合工作提供经验借鉴。

Integration of healthcare services and fusion of medical care and prevention are important direction and focus of deepening reform of China’s medical and health systems.The management system and operation mechanism of the integrated primary medical association has been explored by Bao’an Central Hospital of Shenzhen city,and the model of“Two Integration,One Collaboration”has been established,namely integrated development of hospital and community health center,integrated development of medical care and prevention,collaboration between general practitioners and specialists.Such model achieves a full closed-loop health management for chronic diseases and elderly patients,and the improvement of the service capacity and level of community health centers.As a result,it is widely recognized by residents and professional peers.This paper introduced the main measures,practical effects and experience of the model to provide reference for the integration work of medical care and prevention in the national primary healthcare institutions.
